有战友问起我的checklist，现在附在下面，希望有所帮助，攒攒人品
除了需要填的I-485的表格以外，还需要：
ADJUSTMENT OF STATUS CHECKLIST
EACH PERSON SEEKING TO ADJUST THEIR STATUS MUST PROVIDE THE FOLLOWING:
• Six (6) photographs as per the enclosed instructions (taken within
30 days of the filing of I-485).（我同时申请了AP, EAD，所以需要6张）
• Sealed Medical from an approved civil surgeon on the attached list
using Form I693. (NOTE: You will need vaccination records or the doctor
will give you vaccinations that are medically appropriate for your age);
Note: The results of the medical examination are generally valid for only 12
months. Applicants should schedule the medical examination as close as
possible to the time they file for adjustment of status.
• COMPLETE copy of your passport, including visa and I-94 even if
nothing on the page; Form I-20 and OPT card (if any), Form IAP66/DS-2019 J1
Waiver Approval (if any) and all Form I-797 approval notices;
• Original Employment Verification Letter (for the Principal Applicant
);
• Copy of long form of birth certificate (with translation if
necessary);
• Copy of marriage certificate (with translation if necessary) and
pictures of you and your husband;
• Copy of termination of prior marriage(s) (with translation if
necessary);
• Signed copies of the last three years of U.S. Federal tax returns (
if applicable);
• Copies of your W-2 forms for the last three years (if applicable);
• Copies of your six (6) most recent pay stubs.
**Any document not issued in English must accompany with an English
translation and an affidavit by the translator (please email me for sample
format of the affidavit) by someone who can certify his/her proficiency of
the language and English.**
**If your marriage is less than 2 years, you need to provide documentation
of bona fide marriage, such as the birth certificate of your child. | H*7
