昨晚给SSA发信问了一下换SSN卡的事,得到的回复如下(总结:申请replacement of
Social Security number (SSN) card,填SS-5表,去local office递交):
If you are now a U.S. citizen or if the Department of Homeland Security (DHS
) has granted you a change in status that changes your work status in the
United States, we suggest that you apply for a replacement Social Security
number (SSN) card. Your replacement card will have the same number as your
current card.
To apply for a replacement card, you need to complete Form SS-5, available
for download at the following Internet address:
http://www.socialsecurity.gov/online/ss-5.html
Or, you may obtain Form SS-5 by calling 1-800-772-1213 or by visiting your
local Social Security office or card center.
Along with Form SS-5, you will need to submit evidence of your identity and
current authorization to work or Certificate of Naturalization from DHS or
your passport. Your immigration document may be acceptable as evidence of
both your identity and authorization to work in the United States.
The acceptability of a document must be evaluated on a case-by-case basis,
taking into consideration the applicant’s situation and exploring what
evidence is available for the person.
All documents must be either originals or copies certified by the issuing
agency. We cannot accept photocopies of documents. We need original
documents or copies certified by the custodian of the record. Notarized
copies are also unacceptable.
We suggest that you take your application and immigration documents to your
nearest Social Security office or card center rather than mail them. You
can find that office or card center by using our online office locator at
the Internet address below:
http://www.socialsecurity.gov/locator/
We must verify your documents with DHS before we issue a replacement SSN
card. Most of the time, we can quickly verify your documents online with
DHS. If DHS can’t verify your documents online, it may take longer to
issue a card. We are working closely with DHS to minimize delays.
To read more about evidence requirements, access our Web page, “Documents
You Need for a Social Security Card,” at
http://www.socialsecurity.gov/ss5doc
For more details, please see the answer to the Frequently Asked Question (
FAQ), “How is SSA protecting Social Security numbers?” This links to a
Frequently Asked Question (FAQ) at the following Internet address:
http://ssa-custhelp.ssa.gov/cgi-bin/ssa.cfg/php/enduser/std_adp
Note: The Social Security Administration uses the U.S. Postal Service for
all mail. We cannot track documents (Social Security cards, birth
certificates, etc.) once they have been mailed.

When an individual previously issued a Social Security number (SSN) card with “NOT VALID FOR EMPLOYMENT” or “VALID FOR WORK ONLY WITH DHS AUTHORIZATION” printed on it has been granted permanent residency, he or she can apply for a replacement SSN card that reflects the new immigration status.
An alien granted permanent residency status is eligible to be issued an unrestricted SSN card and will retain the originally assigned SSN.
An alien applying for a work-authorized replacement SSN card must present evidence to establish identity and employment authorization.
When requesting an SSN card, the documents presented, as evidence must be either originals or copies certified by the issuing agency. We cannot accept uncertified or notarized photocopies as evidence.
The documents acceptable as evidence of identity are based on three factors: (1) the applicant’s age, (2) the applicant’s citizenship/alien status, and (3) the relative value of documents.
Primary Identity Evidence for an alien:
-- Form I-551, Permanent Resident Card (includes temporary I-551
stamp/machine readable immigrant visa (MRIV) in combination with an
unexpired foreign passport when the I-551 Permanent Resident Card
has not yet been issued)
Note: Document must be unexpired
RM 10210.405 Evidence of Identity for an SSN Card:
http://policy.ssa.gov/poms.nsf/lnx/0110210405
RM 10210.420 List of Documents in Priority of Acceptability for Use as Evidence of Identity:
http://policy.ssa.gov/poms.nsf/lnx/0110210420
Employment Authorization Documents (unexpired):
-- I-551 (Stamp or card)
-- Machine Readable Immigrant Visa (MRIV) with temporary I-551 language
Documents that establish lawfully admitted for permanent residence (LAPR)
-- I-551 card
NOTE: Some LAPR aliens have conditional 2-year I-551s (Alien Registration Receipt Card), subject to the limitation that they must apply for removal of the conditional basis 90 days before the second anniversary of the admittance date to the United States . A conditional I-551 is identified by an expiration date 2 years later than the admittance/adjudication date is acceptable proof of permanent residency. For SSN purposes conditional resident aliens are treated as aliens lawfully admitted for permanent residence.
-- Temporary I-551
Temporary evidence of immigrant status is issued for an alien to use
until the permanent I-551 card is received. This may be a stamp in the individual's passport or on the I-94.
-- Machine Readable Immigrant Visa (MRIV) with temporary I-551 language
Note: The MRIV shows the alien's identification number (“A” number) as the “Registration Number” in the upper right-hand corner of the MRIV.
The MRIV in an unexpired foreign passport that shows the statement “UPON ENDORSEMENT SERVES AS TEMPORARY I-551 EVIDENCING PERMANENT RESIDENCE FOR 1 YEAR and is endorsed with an admission stamp is the same as a valid Temporary I-551 stamp and is valid for one year from the date of endorsement by the admission stamp.
Example:
If the admission stamp shows a date of entry into the U.S. as January 4, 2012, the expiration of the temporary I-551 status will be January 3, 2013. The expiration date on the MRIV should not be used to determine when the temporary I-551 status expires. The expiration date on the MRIV only shows how long the MRIV is valid not how long the alien's temporary I-551 status is valid.
RM 10211.025 Evidence of Lawful Permanent Resident (LPR) Status for an SSN Card:
http://policy.ssa.gov/poms.nsf/lnx/0110211025
We cannot accept an application filing receipt or notice of action as evidence of an immigration document, or an immigration document with a “valid from” date in the future.
Exception: We can accept an expired I-551 card accompanied by a Form I-797C (Notice of Action) stating, “Your conditional resident status is extended for a period of one year as evidence of LPR status.
Note: An alien applicant can use the same unexpired work authorization/LPR document as proof of identity.
Form SS-5, “Application for a Social Security Card,” is the appropriate form to apply for a replacement card, and it can be obtained by calling our toll-free number, 1-800-772-1213, or download from our Web site:
http://www.ssa.gov/online/ss-5.html
To read and print Form SS-5 at a personal computer, you will need the Adobe Acrobat Reader. The software is available free of charge from Adobe s Internet server:
http://www.adobe.com/prodindex/acrobat/readstep.html
Take the Form SS-5 and required documents to the nearest Social Security office or Card Center.
You can obtain the address and directions to the nearest Social Security office from the Social Security Office Locator, which is available on the Internet:
http://www.ssa.gov/locator
Applicants who live or receive mail in the Bronx, NY, Brooklyn, NY, Queens, NY, Phoenix, AZ, Orlando, FL, Sacramento, CA, Philadelphia, PA, or Las Vegas, NV area should apply at their local Social Security Card Center.
Individuals residing in the Minneapolis/St. Paul, MN area should apply at the Twin Cities Card Center .
Card Center Information:
http://www.ssa.gov/cardcenters/cardcenterinfo.html
We treat an application brought in by someone other than the applicant as a mail-in application when the applicant has signed the application and provided the required evidence.
When an application is received by mail or brought to the office by someone other than the number holder, the photo on the picture identity document cannot be compared to the applicant. Therefore, unless the applicant is present and the photo can be compared to him/her, a picture identity document must also show the person’s biographical information, for example, age, date of birth, or parents’ names, that can be compared to the data on the application and the SSN record.
Normally, an SSN card should be received in the mail within two weeks after the application and document(s) have been received and verified.
However, when an alien requests an SSN or replacement card, we must verify his or her documents/current status with the appropriate bureau of the Department of Homeland Security (DHS). If the initial online verification is not possible through the Systematic Alien Verification for Entitlements (SAVE) system, we may need to send a Form G-845 to the appropriate Bureau of DHS for manual verification.
Social Security field offices can submit an additional electronic online verification, which with the majority of applications eliminates the need to send a Form G-845 along with a copy of the applicant’s immigration documents.
However, some applications still require a copy of the immigration document to be sent to the appropriate bureau of DHS along with Form G-845 for manual verification.
Our offices can also assist applicants by issuing a notice acknowledging the SSN application while documents are being verified.
RM 10205.215 Acknowledgement Requested for an Application for an SSN Card:
http://policy.ssa.gov/poms.nsf/lnx/0110205215
All cards are mailed from Social Security Headquarters in Baltimore , Maryland to the postal address provided on the application.
Individuals can contact the U.S. Citizenship and Immigration Services (CIS) by calling the National Customer Service Center toll-free number, 1-800-375-5283, or visiting a local CIS office.
You can locate CIS offices on their Web site:
https://egov.uscis.gov/crisgwi/go?action=offices
Before going into a local CIS office individuals first make need to make an INFOPASS appointment:

You can speak to our toll-free number representatives from 7:00 a.m. to 7:00 p.m. on business days. People who are hearing-impaired may call our “TTY” number, 1-800-325-0778, between 7:00 a.m. and 7:00 p.m. on business days.
Our 800-number telephones generally are less busy during the early morning from 7:00 a.m. to 9:00 a.m. and in the late afternoon and evening from 5:00 p.m. to 7:00 p.m. To avoid busy signals and extended wait times, we encourage people to call during these times. Our busiest times are during the first week of the month and on Mondays and Tuesdays throughout the month. Some services are available 24 hours a day through the 800 number automated menu.
Callers from Alaska , Hawaii , American Samoa , Guam, Northern Mariana Islands, Puerto Rico , and U.S. Virgin Islands may receive different “live” service hours. However, automated services are still available 24 hours a day, 7 days a week.
