并发症，例如：消化道和胃连接处变得很薄容易破裂引起大出血。现在医生建议：
1.保守治疗：只是吃一些保肝的药，不理会易出血的地方，但平时生活必须要格外小心
，不能用力（甚至入厕时都不能使劲），以防引起大出血
2.做手术：给易引起大出血的地方装支架/套管，但是支架或套管也会有脱落的风险.同
样会引起大出血
所以想问问版上的医生们，像我小姨这种情况有什么好的治疗方案。或有什么好的医生
可以推荐，无论是在美国的或是在国内的都行！

A couple of things need to be considered.
Etiology (what caused the cirrhosis) sometime matters.
It also depends on the severity of cirrhosis and a lot of blood markers need
to be checked for assessment such as CBC (look at anemia and platelet count
), PT-INR (liver is the organ to produce coagulation factors. If this is
affected, combined with low platelet count, patient is at risk for bleeding
in addition for portal hypertension), and liver enzymes as well as other
organ function as liver disease could affect kidney function.
I don't know how severe the cirrhosis your sister-in-law is, as the
prognosis could be evaluated by Child-Pugh score. Child-Pugh score was
originally designed for assessing the prognosis of cirrhotic patients
undergoing surgical treatment of portal hypertension. Another evaluation
system, MELD score, could be used to assess the prognosis of cirrhotic
patients undergoing transjugular portosystemic intrahepatic shunt (TIPS).
Those score also help to determine if liver transplantation is an option.
Most university hospitals have very good liver program and you can search
the university close to you. |
