these definitely look like 1099-MISC situation
I've never received 1099-MISC that is less than $600, and I've never
reported them

bonus?
I forgot to include 1099-INT income from chase few years ago, and received
this from IRS
http://www.irs.gov/Individuals/Understanding-Your-CP2000-Notice
it was a simple form to fill out, paid a little bit interest, not a big deal

citi used to issue 1099-MISC for AA miles, but that's different.
brokerages usually do it
