发帖数: 323 | 1 My daughter has a new doctor this year and we asked the doctor to give us
the state required vaccine form required by school from this year. The
doctor got my daughter's shot record from her previous doctor and said my
daughter still need one shot. Sorry. I always can not remember the names of
shots.
This recalled me one thing from 2 years ago when my daughter was seeing her
previous doctor. That time, the nurse gave my daughter a few shots and
included that on her shots card. But I forgot to ask the blue form required
by state from her before I left the clinic. So I came back to the clinic in
30 minutes and that nurse had left already. So amnother nurse helped me and
told me the original nurse made mistakes filling my daughter's card so she
redid it for us and told me my daughter will not need anything until 18 yrs
old. We didn't go to see this doctor for almost 2 years after that since my
daughter has been health.
So now I don't believe what that nurse said is correct or not.and am afraid
that they may had messed up my daughter's shot record to reflect the fact.
So is this a big problem? Do we need to do tests to find out what shots my
daughter actually had already?

Actually I was feeling something wrong when that nurse was correcting the
shot record 2 years ago. I am afraid the first nurse made the mistake and
the 2nd nurse reworte the shot record without knowing whole things. I didn't
ask because I always can not understand these shots' name.
