If you forgot again, then after oath - you have to go through court to do

that was the case a couple of years ago when I did it.
Going through the court system doesn't take a year. You just need to go to
a local court, schedule a time, and the judge will issue a court order for
the name change. Then you need to notify banks / employer / social security
/ etc. Took me about 2 weeks.

to go through the court process.

when we did our naturalization several yrs ago (not in CA), my wife changed
her name during interview, she did not need to do anything else, name got
changed.
I did not change, but after oath, I asked the people who was working there,
how to change name (just out of curiosity), and was told to go to court and
it's a long process, about a yr.
for LZ, I guess she only need to see during oath what name is written down
on that sheet, if
