（1）ACS，这个本来看起来不会有什么问题，因为胸痛待查不可能不做系列心电图和肌
酶谱，但是也存在很致命的暗礁：
a.是开出心电图Q6hour, 但是病人收到病房，你却在急诊室忙着收病人，于是就忘记了
跑到病房 follow up 心电图Q6hour！这一段时间病情有可能急剧变化，譬如由NSTEMI
变成STEMI，需要立刻上心导管，如果你漏掉了，那就是很严重的事故。所以，新
INTERN的 duty check list 很重要，一定要按时follow up病人，不能够认为收入病房
就万事大吉。
b. 糖尿病患者或老年人，痛阈很高，心梗或PE可能没有胸痛，但是会表现为呼吸困难
。所以，这些患者的呼吸困难，等同于胸痛对待。
c. 常看到“order EKG, cardiac enzyme to R/O ACS” 的医嘱，其实这反映了一个概
念性错误。因为 即使EKG变化不明显,、cardiac enzyme正常，也可能从临床上诊断
unstable angina。关键是UA也是ACS的一个亚型，要按 NSTEMI 处理，给 full dose

I think the diagnosis of Cardiac Tamponade can be made even without Echo in
this case. Cardiac Tamponade is a clinical diagnosis.
This patient is hemodynamically unstable, isn't it urgent to tap some water
out to relieve the pressure to heart before any diagnostic test?
Of caourse the term paracentesis is not a good one, if this option
change to pericardiocentesis, which one would you choose?

得真的好快，我在Med Surg已经干了快两年了。最近申请Telemetry Floor，被收了。
我们医院据说以Cardiac Care著称，而且我当年申请过他们的New Grad Program却没能
拿到offer，所以我现在还是很高兴的。毕业后到找到工作前那段时间我自费拿了个
ADLS，正好这个月要过期了，Cardiac Floor可以有免费的Training，终于这次不用自
己掏腰包 :)
上来向大家汇报一下。谢谢分享我的喜悦。也请有Tele经验的RN们谈谈Cardiac Care的

发帖数: 1872 | 10 这两天又一个assignment， 病人fresh LVAD post op day 2, drive line 在白天PT的
时候被意外拽了一下，开始出血。我接管的时候，出血越见严重。这些只是表面症状。
几小时后病人出现呼吸加快，但血氧还正常，没有用氧气。MAP 没有明显降低。 报告
护士胸口疼痛，有压力，一半常见在开胸手术后几天。 半夜，LVAD 度数异常， flow
正常，但是pulse index 开始下降。 和一个老护士讨论， 怀疑是cardiac tamponade.
病人原计划转stepdown in couple days， PA 和 IJ 都已remove， 只有PICC。 我建
议NP和MD 胸片， PICC CVP （for reference only）和ABG。结果CVP升高，lactate升
高，尿量极具降低，肾功能急剧下降，胸片显示巨大的心脏轮廓的尺寸（baseline，

昨天，这些人折腾了一天，白天，晚上皮啦啦，也不知道谁把谁杀了，谁把谁带走了。
反正我先生和孩子被绑架了，就不知道是不是已经遇害了。我本来是打算睡觉的，但他
们头上，地下装电磁波攻击器，让我几乎窒息。虽然我没倒下，但其实他们在我身上扎
的毒针已经蔓延，我能感觉到我的呼吸有窒息的感觉。我想我对他们已经非常仁慈了，
但他们不依不饶地攻击我，欲置我于死地，让我非常生气，特别是肆意伤害我家里的人
，绑架我先生和孩子，而且有许许多多无辜的生命毁于他们之手，有必要把他们曝光。
当然我还没分析这里边是那几个人具体攻击我，伤害我的，但他们显然都是一伙的。鉴
于他们伤害我的头部，胸部 和胃部，我就把这几部分的医生都曝光，受害者慢慢去调

position changes such as from lying to standing leads to low cardiac output.
Your brain didn't get enough perfusion as needed.
Dehydration from recent illness can be a reason. Even orthostatic
hypotension could be divided into neurologic and cardiac sources. You could
go to your PCP to evaluate situation. Anemia for young females can also be a
cause.

血压身高体重外，还测了心电图。医生过来听了娃心脏，说没事，然后说放心起见，让
我们做个ultrasound。最后木有啥问题。
最近收到账单，恕我孤陋寡闻，短短一小会儿的visit，一个心电图+一个超声波，要将
近8000刀。贵也就算了，关键是账单有点匪夷所思，弄不明白，求专家瞧瞧：

有几个疑问：只做了两个检测，为啥帐单会有四项？Ecg貌似是心电图，ekg，echo是什
么呀？哪个是超声波？
还有，我确定当时没有用任何药物，为何会有medicine一项呢？
有点怀疑是不是把帐单搞错了，可是打了好多电话，问不到，医生永远都不在，保险公
司说我们按照帐单来报的，医院的billing Department说医生给的就这些项目。因为保

may not really reveal potential heart problems.
I just happen to read the following post and would like to share.
It is a long read but the gist is that this guy paid $1400 to have a really
thorough tests because he too find casual blood test is just not enough.

时候，低头看305，也就14的pace 。回想比赛刚开始的时候，12的pace都慢得难以忍受。

他，比如电解质紊乱，低钠， 低钾，脱水，代谢性酸中毒等，并不一定是心脏的问题。
我们医院不让我们写death certificate的时候，写死亡原因是cardiac arrest,
respiratory failure。 因为每例死亡都是这样的。我们一般常见的写sepsis,
pneumonia, intracranial hemorrhage之类的。 |

小白鼠跑个长距离，然后把它安乐死，取心肌细胞分析。
转贴如下：总之，没有心肌的病理分析，没有心电图和心动图的分析，不能判断心脏的
功能是不是影响了。但因为心肌酶的增高，表明心肌细胞有死亡。当然要排除其他的导
致心肌酶增加的原因，比如肾功能差，肺栓塞等。
