朋友的朋友的朋友的小孩，想着到美国来看病，有人知道这个病吗？有任何
information，我都会转告她们的。如果有人知道他们应该去哪个医院，也请告知。
（我也不知道如何帮忙，所以发在这里看看是不是有人知道这个病）

示出来，由于他性格比较活泼，我们没有留意，结果3岁开始上幼儿园不到一个月就被
老师叫去开会，要我签Consent Form将他refer到Early Support (我住在英国，这个和
美国的special education相似，只不过是给学前儿童专门用的，排队时间短）。我这
一生都不会忘记那一天，觉得整个世界都垮了下来，但是也是一个新世界的开始。
诊断的过程非常复杂，前后经历了四个月，包括special teacher上门到家里和幼儿园
观察评估孩子，language theripist，pediatrician, psychologist, 和幼儿园的老师
都有参与，正式报告出了好几份，最后在医院当场确诊，我儿子还是非常典型的自闭症
。所以根据我自己的经验，over-diagnosis应该是很个别的例子。
儿子3岁时我离婚，同他有自闭症有一定的关系，但这不是孩子的责任，关键还是他的
父亲没担当。但是这也侧面说明了自闭症儿童家庭所要承担的压力非一般人可以承受。

继christmas在ER待了一天之后，娃又住了三天院，吊水的过程惨不忍睹。
换了两茬人，千辛万苦在ER埋好了iv，当时我就觉得埋得太浅，但是那护士说没关系的
。果然，前天晚上准备再上抗生素的时候，发现iv掉出来了，肿了好大一个包，不知道
什么时候掉出来的，外面纱布包得很牢。
然后重新再上iv，住院部能上的护士全上了，最后连nicu的护士都叫来了，小手小脚所
有的血管都试了，插进去，在里面搅一阵，偶尔摸到血管，然后又滑掉了。就没一个成
功的。我实在忍不住，去跟医生说，能不能不吊了。
娃实在太太太可怜了，现在不光她有心理阴影，我也有了。

和川崎根本就不搭边。
Neuronlastoma是小儿最常见的神经系统肿瘤之一。physical exam发现有limping,
differential diagnosis里列上neuroblastoma也很正常。
WBC and platelet都升高，最有可能是感染后的正常反应。
要是只有WBC高那么长时间，感染和leukemia都要在differential diagnosis 里。再继
续查去排除。

问这些diagnosis code能不能和speech therapy的CPT code一起报销.如果你小孩有吞
咽方面的问题，可以用Dysphagia 787.21, 这是我们去看的一个语言训练师提供的，我

等等。这是行为判断的诊断，属于精神疾病，所以判断有很大的商榷余地。病人得到诊

不久前，才有个报道，california 发现有autism cluster,在高收入，高教育的地区，
难道穷人就不产生自闭症？因为家长紧张，有资源，所以往往主动找医生验证，更加可

It is very hard to make initial diagnosis on brain stem disease just based
on initial MR. Statistically speaking, brain tumor is usually supratentorial
for adults. But it could happen in brain stem. So it is obligatory to raise
the concern for brain stem tumor for the doctor who read your first MR scan
and recommended MR followup. I would say any doctors with formal training
would do so.

good places to go. If you don't have insurance, just to straight to the ER
and tell them you either have "flank" pain or "blood in
urine"; and mention to them during history interview that you had such and
such diagnosis from China. The physician has obligation to take care of
you. They should offer you counseling on insurance issues, including
medicare. it takes a few months to get approved. If you don't have green

If its a private practice, 当然要bill所有的diagnosis啦。有的医生收到你保险公
司的reimbursement就算了。有的就保险不cover的就找你要了。太正常了！
你要是在诊所打一针Vitamin B12, 那就还要再bill你一笔，哦，这个好像听说是bill
as procedure的。
没什么好抱怨的。就象修车年检，年检是年检费，其它修理是修理费用。

出来的，再有钱人很多都破产，即使老百姓不破产，美国最后也会破产。我没有说看几
个病收一个钱。你完全可以收一个病的钱看一个病。不能一开始说看一病，然后偷偷摸
摸多看几个病，然后收几个病的钱。因为很多时候病人并不想看这个病,或者保险公司
不觉得需要。
比如你说的email,phone，follow up，很多医生不会回email,phone,也不会直接拒绝看
这个病人，而是说你必须来office。体检也是，到了一定程度，医生必须说，再说下去
，我要charge钱了。诊断需要劳动力没错，但是在做诊断前必须给病人说明，病人不要

different code.
in the hospital or big practice, u really need to ask for social worker/
supporting team in terms of visit price or hospital charge.
doctors are hired by hospital or facilities to see the pt. We are required
to put every diagnosis code there. We r here to see the pt, asking the
doctors for the price, u are just asking the wrong person:-)

camera in the joint and watch it on the monitor for the doctor to do a full
assessment.
Obviously Lin still feels the soreness in the knee after several days' rest.
There must be somthing more than 'overuse'. But the doctor can't find
anything wrong with it. So that's a good guess that they will do such a
penetrating diagnosis after the season. |
