you deal the cancer caused by genetic lesion? I think early diagnosis would
help a lot, yet this apply to cancer with clear stage progression, for some
aggressive cancer like GBM, early diagnosis won't help much....the proper

expectation in cancer diagnosis.The only example I can think of is BRCA1/2.
Perhaps deep sequencing of circulating tumor DNA will reach clinic someday
and transform the early diagnosis. Cancer research is heavily skewed towards
treatment, which in my view is wrong.
But I think it is unfair to say "癌症分子水平研究对近年的癌症治疗的进展
贡献微小". There are already impressive examples like Gleevec and PD-1/CTLA

clamchowder is right, EKG most common is sinus tachy, and ABG might be
normal, respiratory alkolosis or normal or hypoxia. in a pt with other co-
morbidity, these can be caused by many other reasons, therefore makes the
diagnosis of PE very difficult.
USMLE, only massive PE will cause hypotention and axis change, if that's the
case, what should you do?

mean it won't change to malignancy. with nipple retraction and skin
involvement, that sounds very suspicious. this needs a core biopsy to get
tissue diagnosis. Mammogram is needed not for this lesion, but for
coexisting lesions and the contralateral breast, presumably she never had
one done.
for this mass, no matter what the mammogram shows, you still need tissue
diagnosis, and core biopsy seems the best way to go. fin |

somewhat difficult. I remember when I was an medical student in China, one
of my classmates made a diagnosis of Grave's disease at his cardiac rotation
after the patient was misdiagnosed as heart disease by multiple senior
residents and attendings. He ended up being an endocrinologist. |

The final diagnosis is : Wernicke's Encephalopathy.
Treatment: Thiamine 100 mg iv stat.
Patient dramatically improved after the injection. Her Vitamin B1 level
came back 1 wk later and proved the diagnosis.
Why bradycardia and hypotension, hypothermia? Hypothalamas involvement. But
you need to memorize mammillary body first.
Risk factors? Gastric bypass, poor nutrition, Beer consumption, and giving
D5 without thiamine!
This is not sepsis. Blood culture was negative.

At ER, she was found to be hypoglycemic which is very common to cause MS
changes, so she got the D5 without thiamine which made her worse.
Second, she developped hypothermia which led to a diagnosis of Sepsis.
Bradycardia and hypotension were thought to be septic shock.
Fortunately, I took over the case and made the diagnosis after I had a long
talk with her husband.

this case. Cardiac Tamponade is a clinical diagnosis.
This patient is hemodynamically unstable, isn't it urgent to tap some water

Of caourse the term paracentesis is not a good one, if this option

MCQ---take time to improve, but doable and worths the time you spend-----don
't have to read book first to prepare step 3----it's about the way of
thinking, not about details of knowledge… what is your first thought in 30
seconds and whether you are confident ---- half knowledge is not in CK and
it has been summarized in the question, better to do questions ----do
questions!!!!
