就能找到。清单：
1，定时器，可直接拧接到水龙头上。分indoor/outdoor, line power/battery power
几种。我图省事，买了outdoor, battery powered的那种，大约30刀
2，1/2英寸胶管，可直接拧接到定时器的输出口。
3，1/4英寸胶管。分有/无格子孔两种。我家植物稀稀拉拉，选的是不带格子孔的。买
时成卷，用前需先将所需长度的1/4"胶管拉直，在太阳下晒上半天，让它变直，否则不
好安装
4，Drip emitter，就是顶楼照片里那个红色的东东。参考

一头尖可以插入1/2"胶管，另一头可以套1/4"胶管。分pressure compensated/non-
compensated两种。简易选压强补偿的，那样水流更稳。

首先, 如果喝的是Drip, 我推荐我最喜欢的一款, 这款和市场上,$100-$200的Drip机效

：我们倒是很少轻易给病人上insulin drip
哎呀，所见略同，所见略同，我今天午会后特意到ICU转了一圈，正碰上我比较熟的一
个菲律宾小护士, (我和她熟，是因为她的菲语lastname 翻成英文是“tea land”，全
院非菲裔人士大概就我一个人知道，所以每次见她都要讨茶喝）。这次照例讨茶而未得
之后，就问她每测一次accucheck，外加按insulin drip protocal 调胰岛素，总共要
多长时间，说了半天她才明白，说三四分钟吧。然后我就算她一个12小时的shift里面
，如果每小时测一次accucheck的话，那么我多开一个insulin drip，她就要多干36到
48分钟的活；她每天12小时的shift管三个病人，要是三个病人都上insulin drip的话
，她要多干一个半到两个多小时的活！她听了以后，说：“我怎么从来没这么想过？”
然后就呆住了，恶狠狠瞪着我，让我心里这个不落忍，想多单纯的小姑娘阿，那么任劳
任怨而毫无察觉。于是我就说，要不这样吧，以后你的病人我尽量少开insulin drip，
不过有一个条件，每少开一个，你给我泡杯茶。她又恶狠狠瞪 |

心里惴惴不安
的，心想这下可坏了，不知道哪里又捅娄子了。。 到了办公室后manager从电脑上掉出

incident report,大概是说两个行期前的一天一个病人的heparin drip的drip rate 设
定的
跟系统里填写的不符，系统里写的是1200Units/hr,泵上却是1024Units/Hr,另外系统里
记录的
heparin bolus没有oder的记录。 然后又说下一个班的护士抽了一次ptt/pt/INR 结果

>145, pt也极高，只能停掉了heparin drip换成了lovenox. 好在我老当时脑子还没全
糊涂
（刚下12小时夜班），还记得当时的一些情行。我说记得当时的bolus的量是根据order
中调整
heparin drip rate计算公式算出来的,而且是有第二个人给核实的，不知道为什么会出
现这种情
况。于是manager就把当天的charting 记录掉了出来，看了看记录，发现当时我就这个

发帖数: 722 | 8 现在回顾一下几个争议问题，还是挺有意思的。下面是我个人根据讨论以及新近经验做
出的结论:
一.手术/procedure 的患者，头天晚上的长效胰岛素（Lantus）怎么办？
根据具体病人情况，可从全部停掉到只减1/3剂量。现在最常用的，是剂量减半。但是
一定要注意手术当天出现低血糖的可能性，因为早餐，甚至午餐都停了，尤其有的时候
procedure会延误到下午甚至晚上，这时候连晚餐都要停。然而，Lantus 的作用时间是
持续24小时，这时候就有术前出现低血糖的可能性。
解决的办法是：
(1). insulin drip
(2). 实际上classic insulin drip 耗时费力，遭护士痛恨，而且在我们医院regular
floor 不能做insulin drip. 所以,可以用一个改良的insulin drip，就是 iv D5NS（
or 1/2NS）加tighter sliding scale, 也就是accucheck Q4hour.
这时候需要注意的是，一定别忘记把病人原来sliding scale 的accucheck QACX3+QHS

装的是碰草坪的SPRINKLER， 由于植物有10多英尺高。 我重新铺了两根DRIP TUBLE到

第二个就是把房子周围，都弄上植物。 去年，DRIP IRRIGATION SYSTEM 不工作，有时
候浇水不勤，两棵植物干旱而死。 我买了两个开花了，2-4 英尺高的，常绿。补上。

FILTER漏水。 于是去买FILTER,结果发现，HD只卖FILTER和REGULATOR一体的。 只好买
了FILTER和REGULATOR一体回来。 装上，结果不知道怎么，这次PIPE TO 1/2 TUBING的

feeling to have something stuck in your throat, so for most of ventilated
patients they are put on diprivan drip(the meds that was found in michael
jackson's home), which is short acting and can turn off and effects go away
and patient wakes up right away; if not diprivan, they can use ativan drip,

我朋友以及当年度一起回国的一批小海归，鼓励他们爱校、爱党、爱国，为祖国建设添
砖加瓦。。。因为这个学校也是自己的母校，对那些人还是比较熟的，知道这些人，如
果年纪大点的，孩子都在国外读书或工作，年纪轻的，也都在打听将来孩子留学的途径
。。。

仔细看了一下季报，注意到它的 bookvalue 现在是 $8.6B(好像比过去低了，我忘了过
去是多少）。 但我记得 yahoo 过去的 book value 是 $15-16/share. 刚查了一下，
yahoo 最新 的值是 $13.9/share.
就算最坏的。 $8.6B book value, 减去 $1.9B goodwill, 那还有 $6.7B, 再打 8 折,
worst case, 它应值 $5-6B market cap. 按今天的 收市价 它的 market cap 是
$3.75B. 所以它至少有 30-35% 的上涨空间. $10-$11还是不难（只要 oil stays
above $90).
又去它的网站听了 conference call, 注意到只有 2 个 analysts 问了问题。 可能很多
funds 都不知道今天 release ER 和 accounting restatement. 如果有更多的人

cap
(注意要把drip cap 插到墙防水纸的后面， 就是让雨水顺墙流下时会从drip cap 上流到

糊涂，她血压总是偏低，用midodrine 10mg tid，护士还page我说低，都50多了，我赶
紧给了100ml albumin (ESRD on HD)。等跑过去一看病人很舒服，自己量了一下血压其
实是80多。你们dopamine drip在病房用吗?护士很强大。至于用 heparin drip，估计
就是像逆风MM说的，如果要是有可能做啥手术的，heparin drip比lovenox更容易控制

Can’t say qd (outdated), have to say once daily. Even for prescription.
Can say UTI, 不一定非要说urinary tract infection.
3 vessel CABG的念法是：Triple vessel cabbage, NOT three vessel, or C-A-B-G.
Can say IV push or bolus, which is different from drip.
Can say drip, don’t have to say intravenous drip.
Can say two-D echo, don’t have to say two-dimension echo.
Say D/C or discontinuous, don’t say cancel. D/C stands for discharge too.

在tele/PCU 工作了2，3年，拿了PCCN和CMC，说说我所在医院的tele floor ，给tele
的新人一点点我的经验教训：）
首先是tele的病例：CHF，pre/post cardiac cath pt, various arryhythmia on
various drips, CVA/TIA,respiration distress on Bi-pap/CPAP, stable
ventilated pt with trach.Packmaker insertion. acute renal failure, chest
tube/PCA PUMP， CAPD啥的就不说了，M/S也都有。还有各种各样M/S处理不了，ICU又
不要的病人 ：）
1最需要掌握的是基本EKG STRIP的辨认和处理。这个ACLS已经覆盖了一些皮毛。
2还有基本DRIP的知识，常用到的DRIP有Diltiazem gtt, amiodarone gtt, Heparin
gtt, Dopamine, Dobutamine,Neseritide Nitroglyci |

BP后来测不到了，HR也开始往下掉。

Ventilator AC 24 500 80%, peep 15.
levophed drip increased from 5 to 30 mcg/min,
vasopressin drip added.
bicarb, fentanyl, versed drip added.

Caffe Americano 是基于 espresso 的，不用滤纸，pump press out 的。。。加水冲
淡以后浓度接近 drip coffee，但风味还是有差别。
