这种问题，我工作中也常遇到。很多时候医生扔下个pharmacy consult for pain
control就不管了。我会先查一下，病人24hr的morphine实际用量。因为一般除了
scheduled narcotics 还应该有 PRN dose. 算一下病人实际用量，然后问问病人或者
护士他的pain controll情况，还有看看vitals，判断以下，是不是用得太多，或者不
够。然后morphine --> fentanyl
你 300：1 用得是 po morphine to IV/IM fentanyl. PO mophine to fentanyl
transdermal 应该是 150：1。 这样用你的算法就对了。
再说都给你po morphine 和 fentanyl TD的换算表了，直接用上多省事啊。 |

oral morphine: fentanyl TD=300:1 （背书错了，TD应该是150：1，肌注/IV才是300:
1）
因此：
360mg oral morphine equals to 360mg/150=2.4mg=2400mcg/天=100mcg/hr （芬太尼透
皮）
芬太尼透皮制剂 每3天换新，所以数量是10张
所以答案是 100mcg/hr, #10

1100 痛得不行 上次内检开2cm，换了个非常nice护士，虽然她说随时可以上epidural
，但不知开了多少，犹豫中决定要了fentanyl，几近天堂 终于睡着
药效过了被痛醒，已经是2小时之后，再要一剂fentanyl，没过一会一个住院医生来内
检，1：50大概。开3.5，软化75%,不理想，宫缩pattern也不理想，要加大pitocin用量
到2ml。靠着fentanyl又坚持了一会，药效只持续45分钟，于是2点过到5点再次内检之
前经过了最痛苦的煎熬，老妈还被接过来了。
5点内检之前还想上一剂fentanyl，被刚换的老护士晓以大义，说你如果这样勤的要
fentanyl，不如上epidural了，fentanyl终究还是没有epidural好，而且对bb也有影响
，于是强忍痛楚等内检。
5点过内检居然开5cm，软化80%以上，泥马怪不得那个痛啊！于是跟得奖似的宣布上
epidural。然后就是天堂，哈哈哈。上导尿管有点痛，但能忍受。pitocin剂量加大了
点。之后开始睡觉。
7点过又内检，开7.5cm，95%软化，继续睡，10：30左右内检10指全开，100%软... 阅读全帖 |

司生产干净，纯净的大麻，对使用娱乐大麻的人来说也是更好的选择。

大麻平等。这样一旦娱乐大麻合法，成人可以花同样的钱买干净相对安全的大麻的合法

歌坛传奇王子（Prince）也是因为服用过量芬太尼致死。而根据美国缉毒署（DEA）调
查，大量在美国流通的芬太尼药丸以及制做芬太尼的原料和压药丸机等设备，居然都来
自中国。
据美国疾病预防管制中心（CDC）统计，美国因吸食过量芬太尼致死的人数在2013-2014
年间暴增了80%，相较之下，死于吸食过量海洛因的人数只成长了26%。
根据《华尔街日报》报导，芬太尼（fentanyl）的毒性是海洛因的50倍，去年春天中国
海关查获了70公斤芬太尼和乙醯芬太尼（acetyl fentanyl），由于没有采取适当防护
措施， 6名海关人员事后发病，其中1人陷入昏迷。美国缉毒署表示，该批芬太尼藏在
货柜内，走私的目的地是墨西哥，出货者为了躲避检查，透过5家物流商辗转送到海关
。经过调查，研判该批芬太尼的背后有庞大的毒品分销网络，源头都来自中国，走私到

day shift people come in。我也让这个order过了。
我也没啥办法了(其实可以oxycodone rectally，估计病人也不会同意的)
但是pain pharmacist不让我以后approve meperidine for pain，说seizure has been
reported after single dose meperidine in patient with normal renal function
. 但是呢，她又不给我一个recommendation，我就想说以后又发生酱紫的情况咋办。
那个病人手术之前有没有take long acting opioid/有无chronic pain issue我已经不
记得了。fentanyl patch onset很慢，等onset了天就亮了day shift就来了也就没我的
事情了。

的鸦片类物质好像新近发现的一种化学合成品W18，比吗啡强一万倍，三十年前在加拿

鸦片类成瘾基本上治疗不好，复发率极高，只能终生长期使用鸦片类药品比如美沙酮等

街上买非法药相对来说既便宜，效果又好，例如海洛因，这样就上瘾了。很普遍的一种
情况是病人因某些疾病或者车祸需要长期吃止痛药，后来没钱了或者没保险了就开始用
海洛因甚至fentanyl。我知道一个人车祸伤了脊椎，疼痛难忍用fentanyl patch一直没
事。后来自己上网查发现有很多副作用就不愿意用私自停药了。再后来又发作可是没药
了就用海洛因，过了十几年才戒掉。你去rehab问问就知道有多少人是这样上瘾的。还
有一些是小儿多动症一直吃药，长大后偶然发现加大药量会很high甚至思维更敏捷，就
这么沾上了。
楼主自称医生，但对美国pain management practice一概不懂就别出来瞎指点了。在美
国看过病吧，要refill根本不用亲自去看医生，打电话让renew script就行，医生怎么
赚你门诊费？ |

再加上少量如Fentanyl的吗啡类镇痛剂。镇痛效果好是因为这个方法阻断了痛觉从脊髓
传到大脑。应该比静脉注射Fentanyl更对胎儿没影响。而且分娩时的剧烈疼痛会引起体
内的儿茶芬胺增加，从而对胎儿产生种种不利影响。所以不太理解要舍弃epidural这么
好的麻醉方法。 |

我前面的帖子中提到过一个病人：“20多岁的护校学生，她身上贴有3个100mcg/h的
Fentanyl patch，再加上口服Oxycontine 120mg Tid plus Percocet2-3 tab q4h for
breakthrough pain。我粗略地算一下，她使用的剂量大约相当于每日口服morphine
3000-3500mg或是IV Morphine Sulfate 1000-1200mg/day。她一个人一天的剂量就足可
以杀死我自己加上这个版上所有的班长，可她还是活蹦乱跳的，而且一见到医生就喊痛
，全身痛，12/10。”
这个病人是以前我做住院医查房时见到的，她的具体病史记不清了，好像是为了一个
小手术收她入院的。因为她的吗啡类止痛药不单计量高的出奇，而且当时Fentanyl
patch/Oxycontine都是临床开始使用不久的新药，所以印象深刻。

阿片类毒品成瘾性很高一。尤其是海洛因，静脉注射一次即可成瘾。
阿片类毒瘾非常难以戒除。海洛因可以认为不可能戒除。因为阿片类毒品就是“幸福”
本身。人类的一切活动都是在追求幸福，但是现在没有发现任何其他活动带来的“幸福
感”能够超越海洛因。

fentanyl)和相关毒品及原材料化学品的主要来源地。
16日，中国国家禁毒委员会给美国缉毒局北京办公室发布传真，称以上声明“缺乏一定
数目真实并确诊的病例支持”。
在这封写给美国缉毒局的信中，中国官员敦促美国提供更多证据，来证明中国充当了这
样一个来源国。
中国官媒《中国日报》本月的一篇文章也把这一观点公开讲得很明白。文章称，中国制
造的卡芬太尼(carfentanil)并非在美国摄毒过度死亡案例的起因。
美国缉毒局官员表示，他们的个案工作和调查不断地指向中国。缉毒局数据也显示，每

似，但是核心结构相差很大，合成应该是非常容易的。 |
