references or strategies that will benefit you. You will need to follow the
guidelines. After that, everything should flow nicely.
I followed the guideline two weeks before the test, and got 6 in the test.
I would get 2 if I did not follow the guidelines though.

今天受到国内亲戚的亲情求助。小女孩今年11岁。2010年查出染色体异常(Turner
Syndrome). 染色体检查结果是：淋巴细胞培养，G显带核型分析，在400-500水平，核
型：45，X.
目前医生采取的是在肚脐周围注射生长素的做法，先刺激其增长身高。尔后，等到骨龄
闭合，再服用雌激素。
但是，在治疗过程中，身高的增长不是很理想，智力的发育也有待提高等。小姑娘真的
是好可怜, 希望能提供一点信息，也许可以帮到她的治疗，改变她以后的命运。
因此，想求助这里各位医学达人: 美国目前对这种病有没有最新的治疗方法，或最新的
药物？

preventive care做的到不到位，就是一个指标。五十岁以上，病人有没有做
colonoscopy，只要你的病人没做，说明你的工作没有做到位。 其实你做到85%左右了
，已经是很好的了。但标准是90%-95%的话，还是说你没有到位。
12. 行医保险？ 最基本要知道的tail insurance。病人有两年时间追究医生，保险
就要cover你两年，在你找工作的时候要问清楚。 如果cover，你想走的时候就能走。
如果不cover，你走就要自己out of pocket。
13. 怎么找病源？再好的地方，也要两三年内build up你的病源。 我们不会自己投
资一个clinic，一般加入group，或者被hospital hire。你尽量广而告之， on line等
途径，把你的名气打出去。 最终病人过来看你，病人病人之间相互推荐。如果病人把

retail，就是residency。就在两年，甚至一年前都有学生毕业直接进医院的，但也是
少数，两年前毕业的去得更多。
想问lz几个问题。可不可以分享一下如何organize guidelines， 尤其是有些比较
specialized。 有的时候看了很多paper，做了很多功课，发现原来有guideline可以看
。然后是怎么可以比较快的知道新的guideline出来了。我现在就是靠听别人说的才知
道，或者偶尔收到一些news letter之类的才知道。
现在有什么PGY-2方向还有demand。我知道的是critical care好像不多了。
在毕业之后怎么可以比较好的准备开始residency。怎样做知识上的准备。

时候就是要查资料和靠业内的交流。用dropbox很方便，随手把自己查到过的guideline
,文献存下来。至于新的，你是不是ashp member?如果是，注册他们的群发信，有新的
guidelin出来他们都会及时更新。 |

您提到了医生的动机问题。我相信againstwind是好人。可是你给我的回帖也说“那个
医生是hormone doctor，呵呵，什么背景不言而喻”。所以我也要呵呵了。其实医生也
是人。为了降低自己被诉讼的风险而defensive medicine，这样的研究已经连篇累牍，
不在此赘述。我自己认识的朋友（北美某著名医院的CMIO）就说90%的ＣＴ scan 是不
必要的。
还有就是surgeon们只懂得切除。你把病人refer到他们那里，基本没有别的治疗方案，
就一个字---割。一看结果是疑似恶性，立刻拿出schedule book, 说可以给你安排下周
手术。而且这些surgeon把这个手术说的简单，安全，速效，无痛。在这种情况下，很
多病人都把手术作为唯一的方案，而不再思考。
您提到“甲状腺素片，没有副作用”。可这不等于割了甲状腺没有副作用。甲状腺素片

能问私人问题。比如说你有两个qualifications基本相当的candidates你只能选一个，
你问下面任何一个私人问题都不妥当，都有可能造成没被选上的人告公司。
1。年龄（年轻的对年老的）
2。性取向(straigt vs. homo)
3。家庭状况（比如小孩有几个。多对少）
4。婚姻状况（比如离婚对结婚，结混对单身）
5。父母情况（比如死了对活着，生重病对健康）
6。transgender(this involves toilet use).
Also there is no any business value to ask personal question in recruiting.

一下，完全就是华为想通过tmobile的测试，几个员工伙
计卖力过猛，被杂种们无限上刚了，几条罪状一个是照相，一个是拿出去测量了一下尺
寸。。。另一方面老中的不拘小节的习惯还确实是会被人抓住辫子欲加之罪，何患无词。
还有就是从以下的报道看，华为美国销售团队的老美当时过于简单草率地认了口说自己
员工有问题，埋下了事后今天被tmobile任意无限上纲的伏笔。。。教训！华为美国公
司不要太轻易轻信老美！可怜的是那几个卖力的员工，本来辛勤工作，极具一心扑在工
作上的劲头，结果不但被华为美国公司的老美马上解雇，搞不好还会被刑事起訴，什
么煞有介事的工业间谍，美国对中国人的白色恐怖，真的是歇斯底里，丧心病狂了。
这件事几乎可以下判断了：
完全是华为员工为了通过tmobile的测试卖力过猛。现在铺天盖地无限上纲到疯狂歇斯
底里炒作
了。
强烈提醒：老中在米国， 连一点垃圾都不要从公司带回家，否则歇斯底里白色
