发帖数: 5750 | 1 糖尿病的诊断是根据血糖来诊断（包括随机血糖和/或者空腹）， OGTT是测试糖耐量，
可以看血糖和胰岛功能（分泌Insulin 的量）。
临床上不靠测试Insulin Resistance来判断二型还是一型。 Age Of Onset， 有没有
Ketone， OGTT显示Insulin Level （这里面有很多讲究啦，比如End Stage Of Type2
， Insulin Level也很低， 长期高血糖导致胰岛抑制也可能Insulin Level很低， 但
是临床医生很容易就可以鉴别这些情况，不至于把二型误诊为一型。）。 还有就是成
人迟发性一型糖尿病（LADA， Latent Autoimmune Diebetes Of the Adults), 可以测
试GAD等抗体来鉴别，当然临床表现也有特殊，这需要临床医生根据临床表现判断来
Order这些测试。
总之，临床上不会测试Insulin Resistance。

ICU的病人血糖高，如果以前hgba1c正常的，一般是stress induced hyperglycemia。
人在术后，以及critical care的时候激素，和正常的时候是判若两人的，非常insulin
resistant，这时候要控制血糖，只有用insulin.从最初的宁可高血糖，到后来的80-
140，到现在把goal定在140-180，但是控制血糖从来都是应该努力做到的，特别是对胸
外科手术以后的病人。
insulin sliding scale，是内分泌医生最讨厌的regimen,很难有真正效果，也不符合
科学原理，所以会有楼主所说的前后似乎没有明显效果。但是因为容易使用，非常普遍。
还在ICU的病人，如果血糖持续高于180,一开始应该上drip，然后血糖稳定以后，
transit到subQ insulin.
如果tube feeding, continuous feeding,给regular insulin Q6hr scheduled,如果
bolus feeding的，在bolus之前给sho |

：我们倒是很少轻易给病人上insulin drip
哎呀，所见略同，所见略同，我今天午会后特意到ICU转了一圈，正碰上我比较熟的一
个菲律宾小护士, (我和她熟，是因为她的菲语lastname 翻成英文是“tea land”，全
院非菲裔人士大概就我一个人知道，所以每次见她都要讨茶喝）。这次照例讨茶而未得
之后，就问她每测一次accucheck，外加按insulin drip protocal 调胰岛素，总共要
多长时间，说了半天她才明白，说三四分钟吧。然后我就算她一个12小时的shift里面
，如果每小时测一次accucheck的话，那么我多开一个insulin drip，她就要多干36到
48分钟的活；她每天12小时的shift管三个病人，要是三个病人都上insulin drip的话
，她要多干一个半到两个多小时的活！她听了以后，说：“我怎么从来没这么想过？”
然后就呆住了，恶狠狠瞪着我，让我心里这个不落忍，想多单纯的小姑娘阿，那么任劳
任怨而毫无察觉。于是我就说，要不这样吧，以后你的病人我尽量少开insulin drip，
不过有一个条件，每少开一个，你给我泡杯茶。她又恶狠狠瞪 |

这个老鼠的肌肉是insulin resistance，但是整个老鼠并不insulin resistance。
是个比较奇怪的mouse model。
相对靠谱的解释是说，adipose tissue变得更加insulin sensitive，
然后就起到了compensation的作用。 |

，取出肝，WB 发现pAkt比对照增高。请问这样做合适证明这种老鼠更insulin
sensitive吗？需不需要空腹3个小时，在给insulin？reviewer会认为这时对照和ko老
鼠在不同的进食状态而导致不同的pAkt吗？ 谢谢！
另外，做过给glucose然后每半小时测血糖，30min比对照高，但是2小时后，还是很低
的血糖（跟注射glucose前一样）。 |

receive多少insulin？再想想如果
没给insulin血糖还会不会不变？；）In medical icu, the goal of blood glucose
is 150 to 180. his/her suger is well
controled WITH insuline. there is better outcome with tighter blood glucose
in surgical icu. I guess dr. Liu's long long

1) give insulin as ordered
2) check glucose
3) call doctor with result
做kaplan的时候，question trainer 5, #147，DM 1病人9am cataract surgery,
usually 30U of NPH and 10U of regular insulin at 7am everyday. 问今天怎么办。
按照sick day rules，我选了give insulin and monitor BS q 2-4 hours.
答案是hold morning dose of NPH and regular insulin and monitor BS.
刚好和sick day rules相反。是不是sick day rules只适用于sick day，这样的
surgery/procedure day不能照搬。
有谁知道，能否解释一下。 |

后从上清里面收集（我不需要提纯）？但是我听说用mammalian cell culture去表达
human insulin的话会因为process proinsilin效率的原因产量非常低。

beta-2受体是通过Gs通路起作用，激活Gs,升高cAMP.
Glucagon也是通过cAMP起作用的，也是升高cAMP.
Insulin是降低cAMP.
这样你可以看出来，beta-2是升高血糖，这没有问题。
Insulin抑制Glucogan.但是Glucogan刺激Insulin.
但是你说的最后一个我也不知道。 |

发帖数: 611 | 15 NEJM上有过一片文章，好像是叫nice trial,结果是ICU的tight blood sugar control其
实并不能降低死亡率，反而增加病人低血糖的危险。所以现在ICU的血糖控制不是那么
aggressive了，一般180以下我们就不管它，我们倒是很少轻易给病人上insulin drip
，200多的血糖上insulin drip，accucheck q1h护士会恨死你。continuous tube
feeding的可以用lantus，sliding scale不好，是吃饭的人用的。TPN的把insulin加在
营养液里就可以了。 |

我觉得光用fasting glucose和OGTT， 只能知道一个人是不是diabetes mellitus吧？
是不是还需要做个insulin resistance test，从而进一步确定是type1还是type2？
如果需要的话，临床上一般用什么方法呢？是不是测测fasting insulin & glucose,
然后算算HOMA-IR, HOMA-beta就可以了？还是其他的方法？
还是说不需要考虑insulin resistance的问题？

pharmacy.
My suggestion for you is to take a small vial of your mom's insulin to the
pharmacy, ask the pharmacist which needles you should buy. There's a variety
of needles.
Also, there's a limit to how many syringes/needles you can buy per day, you
will also sign your name in a log for legal purposes.

carb低的食物),直接导致每餐2小时后肚子饿,于是晚上睡觉前也要snack,结果导致有10
-15%的fasting number在90-95徘徊,其他均为80多。现在营养师建议考虑少量insulin
减小fasting number，大家都是多少开始上insulin的？
我现在才28周，自从3周前开始控制饮食体重已减了好几磅，现在130磅重，比正常体重
重了10磅，比怀孕前重了6磅(备孕的时候胡吃八吃，长了好几磅)，这样的体重让我有
点担心了。 |

发帖数: 695 | 22 很奇怪，光午饭超，不属于典型的妊娠糖尿病。我当时是fasting容易超，上了夜间的
长效insulin。后期好了我就没用了。
我觉得你还是能控制的，饭后散步半小时试试吧
另外，你的医生很严格，我的医生是饭后一小时不超140，两小时不超120.我每天扎7次
指头。
当然，insulin没什么副作用，打了能稍微放开点吃，完全不吃米面也不可取，娃的营
养要均衡。

multiple lipids infusion加HFD，
应该可以保证一周内muscle specific insulin resistance吧，赫赫。
另外做clamp很麻烦，普通lab没做过这个的，一时半会儿估计set up不起来。
GTT，ITT凑合测测吧，然后取点muscle tissue，insulin处理一下，再看看Akt就知道
有没有resistance了。 |

不少，比如这是前三个
Cytochrome P-450 CYP2E1 knockout mice
Glucagon-like peptide-1 receptor knockout mice
Toll-like receptor 2-deficient mice
等等还有一堆，除了传统的insulin IGF通路相关的，很多基因我也不知道还有这功能
。所以如果你用这些KO也可能因为他们其他方面的功能吧。 |

如过insulin sensitivity升高的话，30min GTT， glucose 比对照组高似乎矛盾。考
虑ko会不会影响stress level。
最好直接上clamp。 |

the last one month of my pregnancy. Looking back now I feel that if your
other numbers are within the limits, I do not see it a big problem - if I
got to chose again now, I won't take any medication or insulin (btw, my
fasting number is between 100 and 115). Interestingly, my numbers are well
below the internatinal and China limits. My ultrasound doctor told me that
he thinks US doctors are taking gastational diabetes too |
