发帖数: 10056 | 2 You can download the I-693 form and fill in your personal information. Doctor will fill the I-693 his/herself but will use personal information you filled in your i-693. In doctor office, front desk will also give a number of forms to fill. You will be asked for your health history, family health history. But they have their own procedures for the whole exam. You don't need to fast. Take a shower before the visit. Doctor will also talk with you to observe your mental health.
