准备如下材料，寄到基于I-485的地址（看I-765的Instruction即可）
1. Signed I-765 and I-131 Forms
2. Copy of I-765 and I-131 Approval Notice
3. Copy of EAD/AP Combo Card (front and back)
4. Copy of I-485 Receipt Notice
5. Copy of passport information page with last entry stamp page and I-94 (
front and back)
6. Copy of H1B Approval Notice, if any
7. Four passport style photos
