is what I got from my attorney.
▢ Medical exam from a USCIS-Approved Physician.
To locate a USCIS Civil Surgeon, please refer to the following USCIS
website https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator
.office_type=CIV) or contact the USCIS National Customer Support Center at 1
-800-375-5283. Please take your vaccination records with you. The results
of the medical exam must remain sealed for submission with your adjustment
application(s).
▢ Eight (8) photos taken at an experienced immigration/passport
photo shop within the last 6 months. See http://www.uscis.gov/files/nativedocuments/M-603.pdf for photo specifications. (Six (6) photos for children under 15).
▢ A copy of your official birth certificate.* This must be the long
-form certificate that reports both parents’ names.
▢ A copy of your official marriage certificate, if any.* This must
be the certificate issued by a civil authority; religious certificates are
not sufficient for immigration purposes.
▢ A copy of any divorce decree obtained*, if you or your spouse
were married previously.
▢ A copy of your most recent Form I-94, front and back or I-94
information from your last entry into the U.S. (I-94 information retrieval:
https://i94.cbp.dhs.gov/I94/request.html), AND copy of the entry stamp from
your passport from your last entry into the U.S.
▢ Copies of documents to show that you have maintained lawful
status since you began living in the U.S. Please submit copies of all
applicable documents in the following list:
Copies of your current and expired passports (cover and all pages);
Form I-20 for all periods in F-1 visa status (both sides);
Form IAP-66 for all periods in J-1 visa status;
Employment Authorization Cards for all periods of employment in Optional
Practical Training;
The I-797 Approval Notices for EVERY previously approved employment petition.
* All documents submitted to the USCIS must be in English or accompanied by
a signed, certified English translation. See attachment with details on
translation certification. |

我们的律师也只要求6张：
Six (6) photographs. Photographs: 2 photographs are required per
application ( I-485, I-131 and I-765). Please clearly print your first and
last name on the backside of each photograph in pencil. Please refer to http://travel.state.gov/passport/pptphotoreq/pptphotoreq_5333.html.
