d) 1 passport-style photo of US LPR with full name & Alien# on back;
e) 1 passport-style photo of intending immigrant/foreign spouse with full
name on back;
f) Copy of the front and back of US LPR’s permanent resident card;
g) Copy of US LPR’s foreign birth certificate and Social Security Card ;
h) Copy of Intending immigrant’s foreign birth certificate;
i) Copy of Intending immigrant’s passport pages: Biographical page, Visa
within last year;
j) Copy of Intending immigrant’s current Form I-94 & I-797 H1-B Approval
Notice, evidencing that she is in the U.S. on a valid nonimmigrant status;
k) Form G-325A signed by US LPR;
l) Form G-325A signed by intending immigrant.
m) Copy of US LPR’s foreign notarial certificate of divorce;
n) Copy of marriage certificate;
o) Evidence of bona fide marriage:
• Copy of lease verification showing joint tenancy of common
residence;
• Copy of joint credit cards and shared accounts;
• Copy of joint auto-insurance & renter’s insurance;
• Copy of life insurance verifications showing spouse as
beneficiary;
• Copy of flight tickets to visit each other when long distance;
• 10 selected photographs documenting relationship and marriage.
II. Form I-485 Application to Adjust Status as Permanent Resident
a) Form G1145 E-Notification of Application Acceptance;
b) Check in the amount of $1070: I-485 filing fee of $985 and Biometric
fee of $85;
c) Form I-485, Application to Adjust Status as Permanent Resident;
d) 2 passport-style photos of intending immigrant/alien spouse with full
name on back;
e) Copy of marriage certificate;
f) Form I-693 Report of Medical Examination and Vaccination Record;
g) Copy of intending immigrant’s foreign birth certificate;
p) Form G-325A signed by intending immigrant;
q) Copy of Intending immigrant’s passport pages: Biographical page, Visa
within last year;
r) Copy of Intending immigrant’s current Form I-94 documenting lawful
admittance into the U.S.;
s) Evidence of the applicant’s lawful status in the United States;
t) Form I-864EZ, Affidavit of Support:
• Form I-864EZ filed by Sponsor/Spouse;
• Copy of the front and back of Sponsor/US LPR’s permanent
resident card;
• Copy of Sponsor/US LPR’s passport biographical page;
• Copy of Sponsor/US LPR’s birth certificate and Social Security
Card;
• Copy of Sponsor/US LPR’s letter of employment;
• Copy of federal tax returns for 2012, 2011, 2010 with W-2s and
Form 1099s;
• Copy of Sponsor’s paystubs from January to July 2013.
III. Form I-765 Application for Employment Authorization
a) Form G1145 E-Notification of Application Acceptance;
b) Form I-765 filed by Beneficiary;
c) 2 passport-style photos of intending immigrant/foreign spouse with
full name on back;
d) Copy of Intending immigrant’s passport pages: Biographical page, Visa
within last year & Form I-94;
e) Copy of Intending immigrant’s last EAD (front and back);
f) Copy of Intending immigrant’s foreign birth certificate.
IV. Form I-131 Application for Advance Parole Travel Document
a) Form G1145 E-Notification of Application Acceptance;
b) Form I-131 filed by Beneficiary;
c) 2 passport-style photos of intending immigrant/foreign spouse with
full name on back;
d) Copy of Intending immigrant’s foreign birth certificate;
e) Copy of Intending immigrant’s passport pages: Biographical page, Visa

necessary documents here and there!
