是直接叫 first name?
还是叫 Dr. Last name 表示尊重呢？

Really depends.
Most of the time I started with calling them by their Last names but ended up calling them by their first names. Pay attention to how they are introduced and how their colleagues call them.
Starting with Dr. Lastname is always safe if they are introduced as Dr.XXX. Sometimes they will tell you to relax and to call them by their first names.
