printed out 186 Form, and found out there were 2 types. Type A needs to be
signed by applicant and medical school official, Type B needs to be sighned
by applicant and Public Notary. But there is no form has the signing places
for both medical school official and notary. Do I need to ask ask medical
school official and notary sign Form A and Form B separately, and mail both
back to ECFMG? Because I was expecting to see a form | t**********s

发帖数: 32 | 5 Thanks a lot for the reply. But I went back and checked my form, in Part A
it only has signing place for applicant, not for notary. In part B, it
needs to be signed by either medical school offical (type A form) or Notary
public (type B form).
The form is labeled as revised in May 2009. I guess maybe they just start
using this kind of new form. I do remember somewhere in the instruction
that I have option to choose either type A or type B form. I need to call
them to find out if both are

Then you must have the wrong form. If you can visit your medical school, you
current form is fine. If you are in the states and your medical school is
in china and you do not want to go back just to sign the form, then you need
to use another form. I filled my form last week, there are places that I
sign, norary can sign and then medical school can sign.

print out the 186, do they ask you to select one from two choices like what
I copied blow form the website:
You must click on one of the links below to access and print your
Certification of Identification Form (Form 186). Use the link that lists the
official who will sign your Form 186.

could provide online certification for my medical degree (forget the name).
If you are from schools in this program, you will have two options:
1) Have notary to sign the form and mail it to ECFMG. Then ECFMG will ask
your school provide certification online.
2) Ask your school sign the form and mail it to ECFMG. They did not say for
sure that you need to come back china and physically be there, but I guess
that is req

When you have completed the application, you will be able to view the Form
186 options available to you:
If your medical school participates in EMSWP, you will be able to select "
Form 186 – Certification by Notary Public," or "Form 186 - Certification by
Medical School Official."
If your medical school does not participate in EMSWP, you will be able to
select "Form 186 – Certification by Notary Public and Medical School
Official" or "Form 186 - Cert

Sorry that I was only able to come back today.
It seems that you have solved your problem. Any way, I chose the following
selection and printed my Form 186. It seems that you chose another selection
. I did that because I do not know if my school have participated that
online service.
