heparin drip and then coumadin
or low molecular weight heparin(fragmin or lovenox）and then coumadin if pt'
s renal function is good.
check PT/INR q weekly in the first month if stable, change to every month.
Need very good compliance and regular follow up. The course of treatment is
dependent on the cause of DVT or PE
