first congrats to him for matching to his top 1 choice. Allergy and Immunology is
a very competitive field. I guess I am the only one CMG he mentioned in his
post and truly I didn't see any other IMGs when I interviewed. It is just a
small sampling pool. But statistically only 10% positions matched to IMGs - that
will be about 10 - 20 positions over the country each year. 麦地乖乖虎said well
about A/I - definitely lifestyle oriented fellowship. Somebody may think it is
boring but hey, that is why A/I is attractive to these "fake doctors" (a comment I
got from one of our tough heme/Onc attendings when he heard my fellowship plan). But that is not always true. The "A" part for A/I is bread and butter and can be somewhat "boring" but "I" part is always interesting and challenging. You
can't raise your family if you only practice "I" part though.
One point I want to emphasize - be prepared to deal with peds patients if you are interested in this fellowship. It is more difficult for people with adult medicine background. But the plus would be there is almost never 20 home meds for you to reconciliate. anyways it is commonly believed that the "happiness" index for the fellows trained in A/I tops fellows in other IM subspecialties but again the sense for happiness may be individually dependent.
My background - CMG graduated from BMU in 2000, green card, US PHD in
microbiology and immunology plus postdoc experience, 10+ papers with 6 papers in microbiology and immunology field, IM residency with哲别as my senior (天下谁人不识此君, 呵呵).
Program applied to: 30 programs. most of them are at Northeast region.
didn't applied to CA programs.
Interviews: 11 programs in total. went to Sinai (where I met 麦地乖乖虎),
Hopkins, Northwestern, UPenn (HOP), NSLIJ, USF, SUNY downstate. didn't get interviews from NIH and MGH. There is no official ranking for A/I programs
but academically my personal ranking for top five in adult A/I is Hopkins,
Northwestern, Sinai, NIH, MGH. These programs have their own
characteristics. it will be very interesting to do some comparison. hopefully I can find some time to comment about these programs like 哲别did for his Heme/Onc interviews last year.
Cancelled interviews: Penn state (Hersey), UMDNJ, UTMB, Baylor.
Finally I prematched to a fast track position in my home A/I program.
General feeling for fellowship application is that the research background
which most CMGs have outweighs other factors. So folks as long as we can get into the residency, things will be easier - but that doesn't mean we can sit and wait. I remember 哲别 had a old post last year talking about how to prepare the
fellowship application. very well said. definitely fellowship is the thing we have to
consider on day 1 when we step into the residency training if we want to go
for it. it depends on the personal interest, prior research experience (very
important. so far most people I heard matched well had consistent research
experience with the fellowship). there are still many things we can do to
wrap up a good application package. But remember things need to be done in a little bit more than one year including several month struggling as an intern first.
so time isn't always enough. No need to waste more time here as 哲别 said
everything last year. I am just following my senior people's track.

一年多时间就够了，你主要看的病人也都是allergy,immunology的病人大部分都refer
到academic center.毕业后如果不太愿意看儿科病人，也可以只看成人了，或者加个年
龄的限制。我以前去一个私人的allergy clinic,平均每天12-15病人，年收入20-25k。
主要收入来源还是allergy shots.大家interview时都信誓旦旦的要走academic,可毕了
业大部分还是去private了。即便是那些top-notched的program.没办法，大家基本都是
