Classic question.
I applied IM last year. I graduated more than 10 years ago. I had no
clinical experience in China or US except I only have a little bit of
volunteer experience. When I was asked this question, I told the interviewer
that "I do not think I will have a problem (No 障碍) coming back to
clinical work. My medical knowledge is the most up-to-date and you can see
from my step scores. Maybe I will have some difficulties at the very
beginning. But I will adjust soon just like when I switched from medical
school to research. I am very confident." Also tell them that you will get
some USCE and OB.
The key is to show your confidence. If you have USCE or Ob or will do so,
this question is even easier.
My feeling is if the interviewers like you, they do not care about your
clinical experience. In this case, it is crucial to show your confidence. If
yourself feels this is a big obstacle, you won't show your confidence. If
the interviewers think it is important and they do not want somebody who
have left clinical work for so long, it does not matter what you say.

...any other input? e.g. research background will benefit the clinical work,
blah blah blah is this a good point to present as well?

If they do not ask you about your research,no need to talk about it. some
interviewers are interested in it and they will ask you then.If they ask you
about it, talk about it in a simple way.
I personally think that research background won't help with primary care much especially at the very beginning. For pathology, it is a different story.
