翻译。

Diagnosis:
Right pleura, biopsy:
poorly differentiated carcinoma with neuroendocrine differentiation. See
note.
Note: Possible primary site includes the lung and remotely, the pancreas.
Clinical correlation is recommended. This case was discussed with Dr. XXX on
12/14/11. My associate, Dr. XXX, has reviewed this case and concurs with
the diagnosis of neuroendocrine carcinoma.
Interaoperative Diagnosis
Frozen Section Diagnosis: Malignant neoplasm (DTS)
Gross Description:
Sections show a poorly differentiated malignant neoplasm comprised of nests
and sheets of tumor cells with high nuclear-cytoplasmic ratio and enlarged
ovoid moderately pleomorphic nuclei. The tumor cells focally contain
nucleoli. Mitotic activity is brisk.The cytoplasm is amphophilic and finely
vacuolated. Occasional pseudo-rosettes are identified. Immunohistochemical
stains are performed with the following results:
Pan-cytokeratin: Positive
CK 7: Positive
TTF-1: Focally positive
CD56: Positive
Chromogranin: Positive
Synaptophy sin: Positive
CK 20: Rare focal positivity
Napsin A: Negative
Calretinin: Negative,rare focal positivity at edge(may be entrapped
mesothelial cells)
CK 5/6: Negative,rare focal positivity at edge(may be entrapped
mesothelial cells)
D2-40: Negative, rare focal positivity at edge(may be entrapped
mesothelial cells)
CDX-2: Negative
ER: Negative
PR: Focal nuclear positivity
BRST-2: Negative
Controls are appropriate. The immunohistochemical staining results are
consistent with a poorly differentiated carcinoma with neuroendocrine

形核， 瘤细胞核集中， 分裂活跃， 胞浆噬中性并有空泡， 偶见假玫瑰花， 免疫组
化显示以下结果：
Pan-cytokeratin: Positive广谱细胞角蛋白+
CK 7: Positive
TTF-1: Focally positive甲状腺细胞甲状腺转录因子1 局部(+)
CD56: Positive
Chromogranin: Positive嗜铬粒蛋白+
Synaptophy sin: Positive突触囊泡膜上的蛋白
CK 20: Rare focal positivity
Napsin A: Negative
