Some said follow ABC, so A.
Some said no pulse, so B.
What's your opinion?
Thanks very much!
7. A 2-month-old infant is brought to the emergency department by
paramedics
after his mother found him not breathing in his crib. He is apneic and
cyanotic; no pulses are palpable, but he is warm. After manually
stabilizing
his neck and positioning the airway, which of the following is the most
appropriate next step in management?
A) Positive pressure ventilation and oxygenation
B) Two chest thrusts followed by chest compressions
C) Establishing vascular access
D) Administration of epinephrine
E) Electrical defibrillation | c******n 发帖数: 492 | 2 B。
no heart beat(circulating blood), the air will be of no use , i think. | s****r 发帖数: 48 | 3 I think we should choose B.
From uptodate:
In the past, clinicians frequently interrupted CPR to check for pulses,
perform tracheal intubation, or obtain venous access. The 2010 ACLS
Guidelines strongly recommend that every effort be made NOT to interrupt CPR
; other less vital interventions (eg, tracheal
intubation or administration of medications to treat arrhythmias) are made
either while CPR is performed or during the briefest possible interruption.
Interventions that cannot be performed while CPR is in progress (eg,
defibrillation, pulse check) should be performed during brief interruptions
at two minute intervals (after the completion of a full cycle of CPR). | H****y 发帖数: 156 | 4 B. no pulse, compression first. | d****y 发帖数: 2180 | 5 呵呵,新的CPR protocol 是CAB:

C-A-B
•Compressions: Push hard and fast on
the center of the victim’s chest.
•Airway: Tilt the victim’s head back and lift
the chin to open the airway.
•Breathing: Give mouth-to-mouth or
bag/mask rescue breathing.
不过这道题是旧的,按照ABC的顺序还是要先给breathing啊。
Cardiac Arrest
• Cardiac arrest in adults is usually
sudden, and the primary cause is
cardiac related. Therefore
circulation produced by chest
compressions is crucial.
• Cardiac arrest in children is
mostly asphyxial which requires
both compressions and
ventilations.
• Rescue breathing may be more
important for children than adults
in cardiac arrest. | d****y 发帖数: 2180 | 6 按照新的 CPR protocol, 毫无疑问是选 B。
如果是旧的ABC protocol呢,该是哪个? |
