Goal: follow up the new trends on cardiology research area.
Materials: the New England Journal of Medicine
Meeting location: Skype
Meeting date/time: 8-9pm once a week on weekends either Saturday or
Sunday. One of them will be the meeting time, and another one will be the
back-up in case.
Meeting Requirements:
1. Straight-forward journal discussion during the one hour meeting time.
Any clinical experience, problems, difficulties, information sharing,and
individual clinical pt discussions will be prohibited during the meeting.
However, the meeting will open the discussion of above informations after
9pm till whenever. The members are free to leave or stay for a free-time
chatting. No requirements or limitations will be implemented after 9pm.
2. I will pick the assignment, which will be cardiovascular-related journal
from the New England Journal of medicine, at least one week ahead of time
for members to read and discuss during the meeting time.
3. The format of discussion will be your choices of reading the journal, PPT
or Word doc for the first couple of months. However, The club will
eventually
require all presentations to be done with PPT format.
4. One member will be assigned as the speaker for the week.
Members Requirements:
No limitations on whatsoever in terms of gender, age,or backgroud.
However, the following requirements are mandatory:
1. Love cardiology and willing to learn more about the trend of this
fascinating area.
2. Willing to spend time and energy to join the club so that we can all grow
together.
3. Willing to share information and give constructive feedbacks.
4. Team player
5. Diligent people
6. Research backgroud welcomed.
I usually open the door to anyone who is willing to learn together so
that
we do not waste our prescious time.
However, I do remain the ultimate explainations on whether any member
will remain in the club for indefinate time based on one's effort,quality of
presentation and even your personality.
Words exchanges, non-professional arguements or personal attacks are
banned in the club.
We will not discuss about the difficulties in terms of how hard it is to
apply cardiology fellowship, what is the point, or other non-sense topics
during the meeting.
I would always want to encourage myself or other people. If we do not
study
or try, we will be primary care physicians; If we do study or try, we are
facing the results of either becoming cardiologists or be primary care
physicians.
There is nothing to lose but everything else to gain!

The first members meeting will be held at 9pm on Tuesday 08/21, which is
next Tuesday. The discussion will be focused on schedule, time, members'
individual request, host candidate, assignment and presentation format.
The first club meeting will start on weekend of 08/25 or 08/26.

To be honest, I have been thinking about it for a while.
During this tough intern year, I understand that everyone is very stressful
and tired in trying to learn new system and routines, including myself.
And I have met a lot of skepicals from different sources about whether it is
worthy or not. I thought over and over and over and over, and I decide,"
what the heck!"
So the competition is tough!
So the chance is slim!
So no one believes that it is worthy of trying!
At the end, it will all become good. Why do not I shoot for the moon? If I
do not make it to the moon, I may land on the star.
I welcome attendings, especially cardiologists would come and give us
guidence. And I understand that their lives are just as busy as everyone
else.
I welcome fellows would drop by and give the group some advices at times.
And again I understand their lives are just as hectic as everyone else.
Everyone is welcomed to come just as I said from last post. The door is
always open. I will have tea and cookie ready if I could.
I always want to remind myself," I will be at least 52 journals short per
year if I do not run this club since we plan to finish one journal a week."
If I do not plan ahead, the time will be wasted with potentially empty hands
by the end of intern year, even the end of residency.

while to improve my clinical knowledge. Learning is never a bad thing.
Cardiology is my favorite and hopefully be my future career too.

One friend asked me whether this club will only recuit the interns and
residents.
This club is for people who have strong interests in Cardiology. I prefer to
recuiting people who have finished step 1 and 2. so it does not have to be
interns or residents.
However, strong interests in cardiology, hard worker and stronger research
or clinic background in Cardiology are highly emphasized in the club. We
will not discuss how to read EKG, 2D echo or general cardiac explaination
during the club meetings because I assume that all members are very familiar
with these basic cardiolgy concepts.
So if in case any general cardiology concepts are your weakness, The club
will be
glad to help you in terms of what materials you need to read to improve.
And
I surely hope that the improvement will happen as early as possible. LOL.
However, I am afraid that these discussion will not be the goal of this club.
The goal this club is to help members to facilitate the cardiology training
in US so that we see more and more CMG cardiologist in practicing.
The 2 months goal is to do high-quality PPT by members, and these PPT will
be shared among memebers for their clinical study and presentations in
individual institution.
I would raise the bar for expectations in terms of the journal reading. So
it is not a social club. I am afraid that I will need to make peole work on
the top of your already-hectic-busy schedule. But the results will be good.
My other goal is that we need to publish clinical papers by members in the
club.
I will really going to emphasize the publications by members in the club.
If BBS will allow me, I will send FYI about the cardiac journal weekly to

In order to facilitate the process so that I do not have to type repeated
questions, Please do the following steps:
1. add my skype ID: emma07251
2. Then on Skype:
1) introduce yourself in terms of cardiology background either in China or
in USA, including clinical experiences and research.
Please include the details like your majors, including cath, EP study,
Pacemaker,
cardiac medicine, so on and so forth, which will help me to assigh the
journals to the members.
2) Your steps
where are you interms of your steps? I really want to know this piece of
information if you do not mind me asking.
I really prefer to recuiting people who have finished both step 1 and 2
because your committment is very important and the club will not be much of
help to you step exams.
3) The first meeting will be at 9pm om 08/21/12.
We will brainstorm in discussion of the club goal, individual goal, journal
choices, assignment and format of presentations.
The individual goal will be assessed and two publications on cardiology area
will be emphasized as each member's goal by the end of next June.The more,
the better.

The results of the IABP-SHOCK II trial parallel those from many recent
outcome trials that have challenged our understanding of the management of
acute and chronic heart failure, including those regarding the use of
pulmonary artery catheters9 and the role of revascularization in ischemic
cardiomyopathy.
##FUTURE
Secondary analyses of data from the IABP-SHOCK II trial may help us
understand the mechanisms of the fa iled response
recognize the opportunity to develop novel and innovative strategies to
treat this condition.
Integrated systems to ensure rapid reperfusion may reduce the incidence of
shock among patients who have had an acute myocardial infarction.11.
On the basis of the findings of the IABP-SHOCK II trial, we must move
forward with the understanding that a cardiovascular condition with 40%
mortality at 30 days remains unacceptable.
we hope that the results of this trial will galvanize a broadly based

Fractional flow reserve-guided PCI versus Medical therapy in stable coronary
disease
##Conclusion
In patients with stable coronary artery disease and functionally significant
stenoses, FFR-guided PCI plus the best available medical therapy, as
compared with the best available medical therapy alone, decreased the need
for urgent revascularization.
In patients with stenosis that were not functionally significant, the best
available medical therapy alone results in an excellent outcome. |
