Just saw 版主Aplusplus mentioned doing private practice under the topic
about Discrimination.
I am a doctor with a couple of years of group practice, thinking about
opening my own practice this year.
Could 版主,or other doctors shed some light? Thank you so much!

group practice is one form of private practice, are you talking about
starting solo practive by your own? my sincere advice, don't.

can you first elaborate a bit why you want to start solo?
the main culprit of a solo practice is coverage, too maintain hospital
privilege, you must secure coverage while you are not available.

You are right on the point. To start a new solo, you need to cover hospital
1. hospital privilege,
2. Get referral. (ER, inpatient),
3. Your patient's satisfaction. etc.
You can share calls with other physicians, this usually is not a problem, especially now we have hospitalists.
I agree with againstwind it is not a good time to start your own practice.
However, I know some friends did very well. One PCP who covers NH as
regular business started his own practice about 2 years ago. At the
beginning, he only saw 1-2 patients a day. Now I heard he is really busy.
He went through group practice twice and finally decided to go private.
To start a new practice is not difficult.
1. Good location, usually professional building or hospital affiliated
complex.
2. Get on Insurance plans, this takes some time, you can do yourself or
have someone do for you.
3. Employees. Good assistants are half of your success.

Solo practice seems give the Doc more freedom in terms of what kind of
patient you want to see.I want to do 100% outpatient, but most of the group
need me to cover the hospital call since it gives the group more income.
Inpatient care is more risky to have malpractice. This is the main reason
for me to think about starting my own practice.
I am thinking that I probably do not need the hospital privilege if I just
do 100% outpatient, and I can share the patient's call with other physicians
. The only shortcoming is that I could not get a relatively long vacation as
a solo practioner, say 1 week or 2 weeks.

With hospitalist service on board you surely can have 100% outpatient
service.
You can have locum to cover you once you are on vacation.
For malpractice issues, covering hospital are better than without. I know
some internists do not regularly go to hospital but they still keep the
admission privileges and admit their own patients if they want, especially
for those patients who are close to you, attached to you, or mad with you. :)

if you work at a desirable area and PCP is not that short, the chance that
you can pick patients are even lower than you have in group practice. most
group have good negotiation with insurance, so they are in the network of
most major insurance, you will have trouble finding patients because of this.
make sure you study the market before you start your own practice. also, you
are looking at a significant investment before you can even see the return,
PCP's reimbursement is quite limited these days if you see medicare mostly,
depending on your age, you need to know how long it will take for you to
get the money back.
if you really want to try practice on your own, take one step at a time,
join an established small practice, say an old physician is looking at
retiring.

you have to borrow a big loan to buy the retiring physician's customers. it'
s better to gather customers by yourself.
the retiring physician will make money out of you...
