昨天看了一个新病人的referral，说三十多岁的男病人，testosterone 升高。我看过
不少女病人testosterone升高，或者男病人testosterone降低，所以这个病例我格外留
意了一下。
看检查报告，那个病人的testosterone level在男性应该是正常的，但是数值边上写了
High。我觉得很奇怪。然后仔细看，那个实验室报告上病人的性别打成了female，所以
用的reference range是女性的，所以就报告是高的。PCP看到数值高，就大笔一挥，
endo referral.
我让他们重复了testosterone，把性别纠正过来，结果就是正常的。

前段时间，医院让我去看一个病人。
第一个hospitalist觉得病人有cushing，所以order dex suppression，早上cortisol
被正常地抑制掉了，第二天接班的hospitalist打电话给我会诊，consult reason:
adrenal insufficiency。 |

testosterone高就make sense了； 不高呢？ PCP怀疑错了？ It would be right time

所以以后用NP/PA干hospitalist的工作不见得会差到哪里。 |

男人年纪大了很多人会脱头或者头发稀疏， testosterone与脱发有关， androgen越高
， 脱发越厉害， 这个也应该对吧？
那么为什么人越老， androgen越低， 脱发反而会厉害呢？

所以我觉得你应该问问consult原因.

而是因为在头皮androgen敏感的部分的follicles因为遗传以及其他因素萎缩。这种脱
发是有特定的pattern的。
年纪大了脱发，是整个头皮（不分androgen敏感不敏感）的follicles开始退化，毛发
周期减短。
不过事先声明，这不是我的领域，呵呵，要看皮肤科的。

Case#1. Cl for depression, as pt withdrawal, not willing to verbally
communicate with primary team. So I went to see pt, found he's pleasant and
talking fine, but noticed him spit frequently. Then I asked "your doc is
concerned you don't talk that much, is there anything bother you that makes
you not willing to talk?" pt replied "doc, you see I have so much saliva,
when I speak I need to spit all the time, so I don't bother to talk" .......
..
Case#2. Cl for depression and suicidal attempt. Went there, pt was on vent,
sedated and unresponsive.....

欲大增， 所以不停的咽唾沫。 看到primary team Docs时， 可能一个个骨瘦如柴，

发帖数: 746 | 27 If you talk about catatonic pt, is a different issue, although pt does not
talk, we can observe affect, peculiar behavior/positioning, echolalia/
echopraxia etc. the case here is that the patient was completely sedated by
medication while on vent, so there's not much psych signs that we could rely
on to assess. Although certainly collateral info can be gathered, but the
main part, patient interview was missing, so cannot perform a full
assessment to give recs. especially this was a SA case, so assess the
suicidal risk is the key, which requires communication with patient.
