标 题: 中性粒细胞减少症 — 我们家宝宝的经历

鉴于zyjyw MM的帖子楼太高，我重新发个帖子说说我们家宝宝中性粒细胞减少症的情况
，希望能给MM一些帮助。
我们家宝宝现在18个月，也是中性粒细胞很低，症断为中性粒细胞减少症。
在她六个月的时候，连续发烧了两个星期，中间去了四次医院，美国这边的医生就是比
较机械化，开始说是virus，让观察，后来就是给开了amoxicillin，可是烧还是没退，
直到最后一次去医院，做了X-ray和血检，发现左肺部大片阴影，当时我们都吓死了，
没有做最后诊断之前，医生甚至怀疑是cancer，我们都崩溃了，最后检查的结果是肺部
囊肿，持续的抗生素治疗，进行了静脉埋管，每天要打两次抗生素，持续了大概三个月
，肺部的阴影终于消失了，中间进行了n次X-ray和CT，小家伙受了好多罪。
此过程中，伴随而来的问题就是中性粒细胞减少症，好多次检查都是0，多一点的时候
最多到500，医生查看宝宝出生时的血液检查结果，发现那个时候是正常的，所以血液
类儿科专科医生给她做了抽骨髓检查，发现原细胞是好的，可能是免疫系统建立不完善
，导致细胞转化有问题，检查了中性粒细胞抗体，发现是阳性，就是说体内产生了一种
杀掉中性粒细胞的东东。
在肺部问题康复后，进行过大概一个月的治疗，每隔一天打一针Neupogen (Filgrastim
Injection)，打针后第二天白细胞就增加，可是过一天后就又降到0，而且这个针特别
的贵，我们因为保险全报，所以也没在意，后来发现账单上显示1000多刀一针。因为除
了血检显示值很低，宝宝其他什么事儿都没有，所以后来医生找了其他地方的专家讨论
，决定停止打针治疗，改为观察，现在我们每个月都去医院抽血检查白细胞的情况。大
概情况是白细胞总量没有问题，中性粒细胞的值还是很低，不过每次有virus或者
bacteria的时候，其他的白细胞都会work，医生为保险起见，每次生病都会给上抗生素
治疗。
最近几次的情况是，每当宝宝生病的时候，血检结果显示白细胞总量和中性粒细胞的值
都是正常的，可是当她recover了以后，中性粒细胞的值又变成0了。这可能是一个恢复
的过程吧，我们希望是。
我上网查了好多好多，好像说年龄越小出现这种情况越容易自愈，一般宝宝四岁以前就
会自行康复了。因为宝宝没有任何的其他伴随症状，所以我们也放宽心了好多，现在主
要是在她有infection的时候会更小心一些。
zyjyw MM 的心情我完全可以理解，当时我也是整天心情很低落，情不自禁的就会流泪
，不过宝宝总是给以最sweet的smile以宽慰妈妈。MM也不用太担心，一般情况下，只要
宝宝还比较active就没什么问题的，希望一切都尽快好起来...

发帖数: 704 | 7 Most likely, this baby has been diagnosed with primary autoimmune
neutropenia, which usually linked with a good prognosis.
Here is what I could copy and paste from internet as a first grade in
Medicine, lol.
Autoimmune neutropenia (AIN) is caused by granulocyte-specific antibodies.
In most cases, AIN is not associated with any other illness or underlying
disease, and therefore is called primary AIN if anti-Granulocyte specific
antibodies are detected, or chronic benign neutropenia (CBN) if there in no
detectable autoantibody against neutrophils (could be false negative).
These two forms of neutropenia are most common in infants and early child
hood. In pediatrics, AIN typically occurs in infants between the ages of 5
and 15 months. If the patient has antibody-mediated neutropenia with normal
marrow reserve, the patient will generally be asymptomatic and at no
increased risk of infection, following a benign course despite the degree of
neutropenia.
Differential diagnosis includes congenital neutropenia, cyclic neutropenia
or secondary AIN. The so called secondary AIN is associated with a variety
of underlying diseases including infection, collagen vascular disease,
primary abnormalities of lymphocytes, and is more common in adults, most
often occurring between the ages of 40 and 60.
AIN is typically characterized by spontaneous remission with disappearance
of autoantibodies. The mean time to recovery may be longer in those with a
demonstrable autoantibody. Therefore, specific treatment of neutropenia is
usually not required. Many patients remain free of infections and require no
or minimal medical intervention.
Observation with serial blood counts is the most common practice.
In general, neutrophil production and tissue delivery of granulocytes is
normal in these patients. Filgrastim (G-CSF) which has dramatically improved
the outcome for patients with congenital neutropenia has limited efficiency
in raising the neutrophil counts in patients with AIN. Interestingly, when
it is administered, it has been observed that neutrophil count corrects
within approximately 12 hours, indicating release of neutrophils already
present in the bone marrow into the circulation, rather than production of
new neutrophils, which takes several days. Nonetheless, the binding of
autoantibodies could wipe out neutrophils in the circulation.

such as a corticosteroids, could predominantly exceeds the effect of
autoantibody binding.
In the late phase of infection, the cell number of neutrophils gradually
decreases due to cell apoptosis and reduced cell output and release. In AIN
patients, autoantibodies could quickly regain their predominance.

发帖数: 9638 | 11 Like what bythesea has posted，the prognosis for autoimmune neutropenia of
infancy is excellent. The condition usually lasts only 2-3 years before
spontaneous resolution, and virtually all patients recover by age 5 years.
