发帖数: 1881 | 1 朋友一岁小孩在国内，前段时间发现翻白眼比较频繁，没有太在意。后来送去医院检查
，发现是可能是视网膜肿瘤。昨天医院CT和MRI的结果出来，确诊是Trilateral
retinoblastoma。小孩的父母非常着急，希望能够保住小孩的眼睛。请大家给出宝贵意
见。
由于这里国内的专家比较多，也有很多已经成功的当上了美国的医生，因此到这里来希
望能够得到一些指导意见。小孩的父母经济条件尚可，如果需要到美国来治疗的话，也
请推荐这方面的顶尖医院。先在这里代朋友的父母和小孩感谢了。

就在前面三个回复里面贴了剩下的所有的片子。请专家自行挑选。）
下面是诊断报告：

Test: Ocular MRI without and with contrast (under 20kg)
Impression:
Right space-occupying lesions at intraocular and sellar region, considering
retinoblastoma (involving optic disc and possible distal area of the optic
nerve), possible combined with right neuroblastoma at the intracranial
saddle area.
Scanning seen:
A raised lesion was seen behind the wall of the right eyeball intraocular,
size approximately 1.20X0.60X1.36cm. Compared with the signal of the wall
of the eyeball, T1W1 demonstrated same signal strength, T2W1 demonstrated
enhanced signal, on T2W1, the signal strength is low compare to that of the
vitreous. With Gd-DTPA enhanced scan, signals from the lesions were enhanced
moderately, but the enhancement was not exactly uniform, (the lesion) at
the optic disc area demonstrated thickening unequally, the signal at the
distal area of the optic nerve behind the eyeball was enhanced slightly, but
demonstrated no enhancement with contrast.
Contralateral orbit and structures behind the orbit are normal. No
significant expansions of the bilateral optic canals were seen in the
scanning. An irregular, lumpy space-occupying lesion was also seen in the
sellar area within the scanning slices, size approximately 2.70X3.20X3.40cm.
The signal was significantly enhanced with contrast.
CT Report
CT Structures: facial plain scan, brain plain scan
Scanning seen: A round mass of approximately 20mmX28mm was seen at the
sellar region, its density was not uniform, punctate high density areas were
seen within the lesion, the CT value was approximately 35-45Hu; there were
no density abnormality seen in the rest of the brain. There were no
expansions seen in the ventricle systems, no displacements seen in the
midline structures.
The size and shape of bilateral eyeballs were normal, an irregularly-shaped
slightly higher density mass was seen posteriorly right intraocular, the
margins were defined, size approximately 5.1mmX11mm, the densities were not
uniform, patchy calcifications were seen within the lesion, retro-orbital
space was clear, the shapes of the optic nerve and the extra-ocular muscles
were normal, there were no bone destruction seen in the walls of the right
orbit. There were no abnormalities seen in the left eye. | m*****a
