Vision Test Requirements
Take a vision test at the DMV office, or bring a valid form MV-619 (Vision
Test Report) completed by a vision care professional. If your vision care
professional is enrolled in the DMV Online Vision Registry, the professional
will notify you that your test results will be entered electronically, and
you will not need form MV-619. If the form MV-619 has a print date of 9/03
or earlier, the form is valid for 6 months after the date of the vision test
. If the form has a print date of 3/04 or later, the vision care
professional marks a box on the form to indicate if the form is valid for 6
months or for 12 months after the date of the vision test. |

字母就好了。高度近视，只要你带着眼镜就能看到。
