describe the employment. Include thename of the person employed, name and
address of the employer, weekly incomeand whether the employment was
specifically authorized by USCIS.
我的情况如下：
2011年用CPT做过full time paid intern.
2012年拿到OPT之后在90天找了一个part time volunteer， OPT将在2013年7月到期。
我的问题：
1.请问我是否要将以上两个工作经历（ paid 和 unpaid) 的都填写？
2. EAD的receipt number 是否是EAD 卡上的13位号码（前三位字母，后十位数字）？
