Sent out to New York Department of Labor on October 18, 2004;
Processed in New York Department of Labor on Feb. 10, 2005;
Shipped to Department of Labor New York regional office on Apr. 8
Approved on Apr. 20
Recieved the approval letter Apr. 25.
