Just as the clamor about Biogen’s seeking FDA approval for aducanumab with
one positive Phase 3 trial started to die down, a Chinese company garnered a
regulatory thumbs-up for its drug, also with one Phase 3 trial. On November
2, Shanghai Green Valley Pharmaceuticals announced that China’s National
Medical Product Administration (NMPA) had conditionally approved GV-971, aka
oligomannate, for the treatment of mild to moderate Alzheimer’s disease.

问题一,目前市面上各种mouse models似乎都是转了human mutated genes,像APP,PS1什
么的.最常见的检测指标是A-beta和NFT的染色. 老鼠也有内源性的APP和Tau, 好象内源
性的Tau所形成的NFT在小鼠中一直无法诱导,那么内源性的APP可以经诱导产生A-beta斑
块吗? 公司能买到针对rodent的A-beta抗体, 说明有这种可能,只是主流的文章似乎都
在关注human gene product.有没有可能用老鼠内源性的APP来做疾病模型?
问题二,目前主流的观点对这个疾病的机理是否偏向于Ca overload? 我们手头现有的
mouse model显示reduced cytoplasmic Ca concentration,类似PSI/PS2 KO 模型,但这

gamma-secretase 虽然直接负责amyloid的成熟/生成,
但是并不是导致amyloid 变性的原因。抑制了它的功能很难说和Alzheimer's
有什么直接的逻辑关系。
而且这个酶在正常细胞里就存在，说明它肯定是有正常生理功能的。
这么广泛的到处去抑制它，不产生不良后果才怪了。 |

最可能的情况就是细胞内部的变性蛋白积累多了，然后负责清理变性蛋白的
系统（这个包括内噬泡和转运这些泡泡的细胞骨架系统）由于某些原因效率
跟不上来，所以变性的东西越来越多，导致细胞产生apoptosis 应答所以该
神经细胞就死掉了。
所以，我认为细胞骨架运输系统和变性蛋白清理系统是两个应该主要考虑的
因素。
那个amyloid 的假说存在很严重的原理上的问题，而且相关的研究根本
就还没有做彻底。不知道为什么Eli Lilly 要抱着侥幸心理花那么多钱去做
相关的药。

in vivo，我觉得研究意义都一般...PrPc那个...没看过那篇文章，不过看了后续
Nature上的讨
论，觉得争议还是很大的说。不过本来alzheimer这领域争议就够多了，也无所谓了哈
哈。我是不相信
oligomer这种东西能有什么receptor，我还是更倾向物理上的破坏什么的。

interim alpha不够的话，等于说没有达到early termination for success的boundary
