的科技进步了。

pacemaker是植入在人体内的，大多是肩荚骨下，pacemaker里面带EEPROM，这个要根据
病人的情况编程，而且还需要测试，植入前还需要消毒，所以这个价钱也不可能等于
4500USD。
植入pacemaker病人需要45分钟，但男护士早上6点多就要起来准备了，主要是擦屁股，
这玩意绝对是技术活，美国除了比如THR这种男护士外，还真没几个胜任的
所以这13万其实不贵，好蛋的民主中国成立了，广大老中土鳖也能享受到这样的高贵待
遇。别的不说，光想想男护士那销魂的擦眼子的动作。。。

An update: My Mom had a DDDR type pacemaker installed. She is on low dose of
metoprolol (25mg daily). As far as I understand, medicines for afib do not
eliminate afib except for slowing down overall heart beat. Is it right? It
is a miracle that her afib also goes away (from the telemetry exam) since
her procedure a week ago! Is it known that a pacemaker sometimes can help
improve afib?

如果病人因为高血压吃BETA BLOCKER，比如METOPROLOL，同时吃利尿药降压，就有可能
因为DEHYDRATION导致肾功能降低，这样METOPROLOL的代谢就受到影响，累积在体内，
导致心律过低。这在临床很常见。类似的还有地高辛，复方丹参滴丸啥的。
这属于可纠正的。
心脏本身的疾病，比如SICK SINUS SYNDROME就是导致心律低的常见原因。
病人就可能需要PACEMAKER了。
心律过低，如果同时发生2ND DEGREE/3RD DREGREE HEART BLOCK，就要注意了，尽快装
上PACEMAKER。否则有可能导致心跳骤停。
家里备阿托品急救。我们在临床上，如果病人心跳只有3，40，很多都要在床边放一只
阿托品。 |

大动脉炎，一个特征：双上肢收缩压一般>10mmHg——这是个命题者给你的Gift, 加肾动
脉杂音，想都不想，“毫秒杀”答案。肾动脉粥样硬化，对老年病人，排在NO1，但题
干已经给了双上肢的压差和肾动脉杂音，直接吃“Gift”, 别想其他！
第二个问题：
Syncope，定义为：“突然的、短暂的意识丧失”，一个常见的原因，就是心脏泵血功能短暂

pacemaker. If ICD is not affordable, how come they have ICD?
How about the one in your original post, Did he pay 130K? Did he pay 13K?
did he pay even 1.3k? I doubt.
I am in a hospital which 70% of patient are poor black patients. they got
free or low cost ICD, CABG,anigogram, kidney transplant, HD. That is why
hospital bill the high price to insurance company.

1. 品牌AICD也好，非品牌的也好，pacemaker也好，去颤器也好，材料价格能有6W美刀？
2. pre-lab, start IV, anesthesia, post OP monitoring加起来的确超过45
分钟，但是总共能有多少？3小时最多了。还是无法justify天价费用
3. 且不说30%是不是成立是个问号
（之前我的手术给保险账单上，保险公司可是一分钱没砍下来）
就算是30%那也够多了。
再说了，账务透明一些，明码实打实标价，不好么？

pain killers.” This is an attitude common to liberals. They know what’s
best and when pushed, they’ll spell it out for you: make whatever

教学这种工作已经算是诸多种类工作中较为灵活的了。楼主现在的问题是，应该以积极
地态度去控制这些疾病，而不是想着缺课怎么样。为了自己安全也是应该的。比如这个
心脏病，大部分如果去装一个pacemaker，严重的话装defibrillator，那就没有大碍。
有些人他就是倔，就老是没事儿没事儿糊弄自己。曾经有个教授就是半夜睡觉的时候心
跳停了第二天就没醒。（当时他老婆就睡在身边后来老婆改嫁牧师牧师到处去讲）。何
必呢？Take whatever needed to control the problem.
至于哮喘，除非你到了邓丽君的地步。我身边哮喘的多了，就是定时喷药吃药，戒烟戒
酒少刺激。都能正常工作。 |
