the CPA application form. How did you deal with this? I never heard of this
before....Did you leave it blank, filled out by yourself, or find a notary
public?

they verify that you are the person that sign it
you can find them at a lot of places, like insurance agent, loan office,
those "chinese" services center
they charge like $10-$20 dollar~
I don't think California application require this though. | G***Y
