Now, I need to submit 485 with that exam result, which I know one vaccine
less. What shall I do? Can I get the vaccine in a regular clinic and put the

All docs have to be from designated civil surgeon. Call your previous civil
surgeon to see if they can re-print the 693 form for a nominal fee and add
the vaccine in. |
