I don't know about the Syntax, but logically:
1st one has a sometimes
2nd one always

Someone compliments you but you have no idea what it means or you can't
appreciate it. This person will probably say " it was meant to be a
compliment". If you ask "what does it mean?" He will say" it means a
compliment".
The first one is more like to explain someone's good intention. Whereas
second one is to explain the sentence itself.
My 2 cents.

right. your interpretation agrees with the environment where Mccain gave his
speech.

what you mean is that 'it's meant' is 'it has meant', isn't it?
but see the original paragraph excerpted from McCain's speech,
"Sometimes it's meant as a compliment and sometimes it's not ... What it
really means is I understand who I work for. I don't work for a party. I don
't work for a special interest. I don't work for myself. I work for you. "
do you still the first 'meant' is a past tense, not a passive voice?

In McCain's speech, "Sometimes it's meant as a compliment and sometimes
it's not" both "it's" are "it is", not "it has". "It is meant as a ..."
is passive voice. You are correct.
"It is meant as a compliment" = "It is intended as a compliment"
What McCain is saying:
"Sometimes [when people call me a maverick] it is [intended] as a
compliment, and sometimes it is not. What [my being called a maverick]
really means is [that] I understand who I work for..."
