You may request a waiver, but it is complicated.
Usually the program will give some extra months, and they will request the
waive after they deem the reasonable period. If you are waived due to visa,
the chance you get in for the next year is slim, only if you got a green

As far as I know:
1. if you have trouble with J, based on mutual agreement of you and the
program, the match result can be void. If only one party (mostly the program
) does not want to wait, they can file claim to NRMP requesting withdraw of
the match result based on 'extreme hardship', and NRMP will initiate a
formal investigation. Until NRMP approves, you CANNOT by fired by the
program. So normally program will give you some grace period (I don't think
it shall be called waiver, the term 'waiver' is specifically used for after
J graduation regarding the 2-year home country serve fulfillment)
2. regarding EAD match. Yes, if you or your spouse is the principal
applicant for GC, majority of the programs will be Ok with you, although
some others do specify EAD not eligible. NOTE: EAD from J-2 status is not
eligible, period.
3. if you are in the process of EAD/GC, then legally you cannot start
residency till you get that. in reality, that might be case by case.
normally program will give you some grace period, as it will be more hassle
and possibly more time consuming for them to file claim to NRMP, and not a
solid base for 'hardship'
