HOW TO SURVIVE IN YOUR INTERN YEAR (BY VICTOR)
1. Familiar with EMR
• Intern’s responsibility and daily work
1. inpt floor: when you enter, check your pts, follow up with lab results
(trend), VS, medication, overnight changes, progress notes(overnight and
morning symptom, PE, lab trend, meds list, assessment and plan, 会诊意见计划
)。During round, report it to senior and attending. Assessment &Plan will be
for yours. 45min for 1pt, intern can take 10 pts as up limit. Once you are
familiar the process, you can shorten to 20mins.
2. GET UP EARLY AND SLEEP EARLY! So you can have enough time to collect
info related to pt complaints and admission. CATCH life-threatening signs:
infection (fever, VS), lab.
3. After collecting info and writing progress notes, then it is time to
round. Do the tasks given by attending if having time during round. If no
time, draw square and cross it once it is done.
4. D/C should be done on time.
5. It is better to enter the program 2wks before the real starting time.
6. You don’t need to be how smart, you need to be very hard working. If
you don’t know then you have to admit NOT KNOWN. BE HONEST! You need to ask
if you are not sure!
7. You cannot have your own assessment plan. But you need to follow pt
changes, complaint, PE, LAB, MEDS very well. Leave a few time everyday to
think of assessment plan. It will benefit your second year!!! YOU ARE NOT A
WRITING MACHINE!
8. 每个人都会犯错误。但是不能犯人命的错误！所以一定要观察好病人和实验室。
如果是预料之中的死亡，那没有事情。但是如果是预料之外的，是要避免。每天要查病
人VS once, check VS trend in past 3days, (add PaO2, glucose [if diabetes,
you need to check everyday], aged over 75, cannot walk due to physical
reasons), check lab results having dramatic changes (low sugar, high K+,
bleeding:Hb, severe acidosis: bicarb, PE(heparin, epcarp), old, pt cannot
walk due to weakness, 多检测，多汇报。Intern is not required to make
decision.
9. 作到以上几点，病人照顾上没有问题！

11. 如何面对上下级和周围的人的关系处理:co interns, senior, attending,
social worker, clerk, fellow, pt, pt family. HOW TO SOCIAL
12. Malignant programs to benign programs. Easy to kick residents out,
big work load. Benign is sure the opposite. Most programs are in between. 如
果给你穿小鞋， 给你challenge, 每个人都在评估你， 没有人你惹的起。我们是来学
习，我们不是来judge别人和program.我是在这三年来成长为attending!当你不确定你
的言行是否有负面时，不要说。
13. 不要抱怨！把工作作好是关键。当睡眠严重不足，严重劳累，被批了一天，再
有人来说你的时候就容易控制不住的时候。世界不是完美的，遇到很多极品从懒intern
到mean attending. 不要在没有建立credit和开始几个月抱怨！！！积极心态，每天
笑嘻嘻，把工作作好！把这个月过去了，可能下个月就好了。如果只对你一个人有意见
，那么要总结。
14. 犯错误，不可避免。小错误，没关系。但是大错误，就要有人保护你，给你说
话。所以，每天你都要交朋友，那么当关键时候，就可能能救你一命。不要怕吃亏，不
要什么都占便宜。尊重senior and attending. 给他们面子。 If you are in round
and have second opinion, you don’t challenge them in front of others. You
need to execute first, but you can talk to this person in person.Explain the
reason you think. Be humble. DON’T CHALLENGE OTHERS! 注意心情不好的时候，
特别要注意。
15. 在临床上没犯大错误，没有得罪人都可以活下来。 在临床上犯了大错，但是
有人保你，那么也可以活下来。
16. 慢慢学会negotiation. Otherwise, 你会累趴下。 如果，上级是没有余地的
，那就别试了。
17. 家庭支持最重要！

1. 如何提高看病人效率：先慢后快。先要搞懂，因为我们什么都不知道。等到搞懂
了，在脑子里形成，自然就快了。形成自己评估病人的套路，然后多试几次，有效了，
那么以后就这么用。取舍很重要。主诉一定问好，体检要focus.但是其他小问题可以放
放， 如小贫血，血压《１８０， 痔疮，小反流。如胸痛病人，容易死人的四大：心
梗，PE, 主动脉夹层，张力性气胸。
2. 怎么准备药物：翻番他们常用的抗生素，和抗高压等商品名。做intern，一大好
处就是多问，没人觉得你傻。提前进去，让他们给你准备。
3. MKSAP书：in service exam 没问题的。
4. 小错误：承认。大错误：请示senior and attending, and write it in notes.
If pt has high BP160,170,180, HR goes up to 120,130, 清醒到confusion, paO2:
<92%.
5. 多请示，多汇报，多动脑。对病人认真负责，不能偷懒，要看病人，要思考，每
天都要检测主诉，查体，实验室。要多问senior, attending，不sure 就要问。多查资
料。
6. EKG要多看，要前后对比看。犯了错误，记住了，就进步。
7. 英语，突击。在生活中没有一句中文。想和说都用英文来轰炸自己。看电视新闻
。反复听１０小时，说两个小时。
8. 不要在program里诉苦。回家找人说去。
9. 常用药能把药名认出来就可以。
10. Uptodate是用来查疑问的。
11. 现在最迫切的不是去看书，而是什么病人来了怎么去问病史，怎么形成鉴别诊
断，怎么把思路穿起来。
SOB, chest pain是两个最多见的症状。把这两个弄好。怎么把从０到有穿起来。如大
部分的肺病，心病， 贫血，anxiety, panic. | J*********4

Victor 讲了他CrossCover别的Co-intern时，体检发现病人有新出现的瘀斑和下降的H&
H，联系病人的肚子痛和Warfarin，考虑到腹腔出血的可能，CT证实是有个2L的大血肿
，救了病人一命。 这些都提醒大家要注意病人的Daily变化和体检，作为Intern一定要
牢牢掌握住Every Detail about Patients, 因为我们的职业关系到人的生死。
