适用人群：yog>10年的老毕业生，没有老板硬挺, 申请内科。
Credentials： yog >15，high scores step 1 & 2, cs pass at 1st attempt, no
step3. Us masters, good publications. Parttime inpatient Observership >1
year. no externship/hands-on experience. Applied to many many programs, 10+
ivs, all community programs. Matched to top choice.
Relievepain的match经写的很好很详细，分析有理有据。相同的就不再重复了。
1. 多申请，广撒网。Community hospital还是我们的主要对象。如果有强劲后台或
grant可以冲一冲university program,是有可能创造神话的。Ny的program要全部申请
。我有一半iv都是来自ny。
2. usce非常重要！这不一定是commercial externship. 有些ob可以present cases
, write patient notes， 如果attending能在lor 里写到通过这些，他认可你的临床
能力，这会很有帮助。我在教学医院断断续续做了很长时间ob，学到了很多, 但是没有
和attending present的机会。所以我
的推荐信虽然写的很强，但没有评价我的临床能力。 面试中被频繁问到if clinical
skill rusty，到后来人家不问我都拿出来问人家。有朋友有hands-on experience 就
没有被问这么多。
3. 让更多pd看到我们的简历。对我们来说，拿iv是最难的，相信我的简历在很多
program都被直接筛掉了。最有效的是去做ob，和pd谈 。acap的nypq ob 最好去做，有
可能拿到他们的iv。Acap的mock iv 也参加，能拿到brookdale的iv。次之找人帮忙递
简历，直接给pd发信。发信要有原因，如果
自己package中有闪光点，但是给筛掉了，可以发。家附近的可以发。泛泛地广发是没
有意义的。也要考虑一下别人的感受，每天被无数雷同的邮件轰炸有可能最后都一律忽
略了。
4. 关于做ob，有人提到先做好的地方，再做差些的，很同意。我们很多人都在教学
医院做research，争取去那里的inpatient做一段时间, ，自己有
意识地练习present case 和临床思维。后来在community program present cases and
answer
questions, 能够impress 人家。因为工作关系，没有做full time ob。现在回想，如
果能去做2周，就能从一个attending拿到一封推荐信，还能见到医院的pd，对申请
会有很大帮助。在好医院ob，尽量争取present cases。You think you can do it
means nothing if no attending knows you can do it. 我做ob的unit没有present
case的机会。如果重来，我会在这个unit学到东西后，争取去别的unit ob， 多找找有
可能有机会。
5. 关于iv，be yourself, but your best self. 多听听别人的经验。但要采用适
合自己的。 我开始和牛人谈过后，第一个面试照牛人的建议说，结果自己都有一种东
施效颦之感。后来还是回到自己。长期做ob帮
助很大。少数program让present a case，能讲得不错，这些program也没有问是不是
rusty的问题。多数program没有让present case, 但是从谈话中可以看出是不是对临床
有了解。对于老毕业生，人家会期待你更成熟，有自己的想法。我说到了我碰到的一些
病人，给我的一些感受。这些可以看到maturity and dedication.
6. 关于research，是有帮助的。但是8篇paper和20篇的区别就不大了。如果有
grant或多篇牛paper一作是很有帮助的。有牛校pd告诉我，如果他们认为你将来能拿
funding，他们可以让clinical suffer somewhat and take you. You’ll pick up in
clinic anyway. 有些新的program更看重research experience. 他们没那么忙，有
hospitalist carry the weight， 而更注重打响名声。老毕业生can try to make a
breakthrough with them, of course, the most important is to have them see
your application.
7. 关于找ob/externship, google一下能找到一些program提供ob，比commercial便
宜，大概500-1000 一个月，听说很多可以present case，这样的就很好。Commercial
的，听到的有好有差。如果attending带很多observer的不好，否则好像还不错。
8. about prematch, got one but didn't take it. Agree with relievepain on
this. Just
add one point. Rank all your ivs including the prematch program on paper. If
the prematch program is in the top 4-5, can take, otherwise don't have to
but be sure you can take
the risk of not matching this year. The majority of imgs matched to their
top three
choices. So if your top three are not niu programs, not courtesy interviews,
and your English isn’t too bad, your chance of matching top three is
pretty good.
有很棒的朋友今年没有上，不用泄气，功夫做到了就能上了。
有老毕业生问现在考是不是太晚了。个人感觉2年内还有机会，所以还不算太晚。但
是的确越来越难，这就意味着要下更多的功夫才能上。拿一个像样的成绩，多做做ob／
externship就差不多了。
About family support，very very important. 这是共度难关的时候，也是能看出
responsibility, maturity and reliability 的时候。支持你走过这段路的人是值得
好好珍惜的。反之，艰难的时候有的spouse can be selfish and undependable，
trust can break in such taxing time.

answer
is no. 没有一定的线。>230能用了。如果可能尽量>240。>250更容易些。220也不是没
有机会。我感觉如果别的方面不吸引眼球，>240 application 被program 注意的可能
性大些。基本原则是分数越低别的方面就要补，比如ob/externship/grant/
outstanding publications/recommendations。
不要被分数frustrate了，但要做更多的工作。
申请前觉得自己已经很努力了。申请过程中碰到一些老毕业生，才知道大家都做了这么
多的工作。所以说没有100%, 只有更多。 | m****3
