have a problem of weak immune system? Are you taking some concurrent drugs
(espeically immunosupressive?). What kind of antibiotics did the doc rx to
you? Take it exactly as prescribed even though you feel sick.

我觉得楼主的case是属于灰色地带，就是说一个对于美国医生来说的结核疑似病例该如
何处理的问题。可能他一辈子也没有见过几个真正的结核，看到一个PPD阳性的就很紧
张，因为当时他的诊所有很多人。如果你的有结核，的确是危害了公共安全。公共安全
比隐私权更重要。这一点你需要理解。医生也不是出于要如何如何羞辱你的坏心，只是
有点紧张的失控了。 |

做了以后, 发现是positive,而且是强阳性,induration>20mm, 但chest xray是
negative,也就是不是active tuberculosis. 医生说很严重,叫我最好联系学校,看需不
需要治疗. 于是立刻联系了学校,health center说只要我的chest xray是negative就没
有问题.
但我仍然十分担心,这会不会造成以后申请H1签证甚至绿卡带来麻烦? 我这样需要不需
要吃药呢? 如果吃药,会有副作用, 不想吃,实在很矛盾. New Jersey的state law有没
有对这个病有规定? |

且一旦吃就要吃半年以上, 否则很容易有耐药性.
主要担心这个病会影响我在美国读书或者以后就业,但现在大家说只要chest xray是

cross reaction from BCG vaccination,
China is considered a high risk country(one of the 7 per CDC),

。听了医生的建议，已经同意用药INH， 九个月的那种。刚吃了几天的药，周围的朋友
和同学都在劝我说不要吃了，那个药对肝的损害比较大（当时医生说这个药副作用很小
）。 朋友们都说阳性是因为我们打过卡介苗造成的。我上网查询过，有人说卡介苗１
０年之后就没有效果了，我很小的时候打过，已经超过了１０年。现在很犹豫是不是要
继续坚持吃药，虽然前几天吃了药，感觉没有什么特别反应，但是想到要吃９个月，还
是很担心会造成什么潜在的危害，特别是考虑到自己应该会在２到３年内可能会怀孕生
小孩，怕会影响到以后小孩的健康。还有听说对肝的影响，也特别担心，因为我小时候
得过甲肝。
很想请大家给点意见，我是不是应该继续吃９个月的药？
卡介苗是１０年以上就失效了么？
还听说现在不接受这个treatment,以后工作的时候还会被查出来阳性，这样会影响找工
作么？

called positive, if your indurated area is 25 to 30mm, it's definitely

the ligament in the spine involved, and TB culture ( TB PCR is more
sensitive) may help the diagnosis. If she did not have BCG immunization

not provide life-long protection against TB. Nonetheless, BCG shot does
decreases the chance of developing acute TB, particularly in kids. This
is why it is one of the most important healthcare policies in developing

latent TB infection, and positive PPD in people with BCG shots are

like public school requires record of immunization in many States. You have
the right not going to that school. I think most schools require PPD
testing, but do not require treatment if there is no evidence to suggest
active TB.

CT检查： 有块
PPD： 强阳性（有鸡蛋那么大）
被诊断为肺结核
吃了两个月抗结核药，CT检查，快没变小。
五月PET检查，支气管镜检，被诊断为肺癌。（大医院的肺科门诊，看的肺癌专家号）
现手术切除肿块和周边淋巴，病理结果：肺腺癌，没扩散到淋巴。
我的问题是，
如果肺癌并发肺结核，手术切除的肿块，医院会同时检测腺癌细胞和结核菌细胞吗？
现在病理结果只说有肺腺癌，是否可以确诊没有结核病。病人家属是否要继续做防肺结
核的措施？ |

听国内一个比较牛的医生讲课，最难诊断的问题就是发热待查了。挺复杂的病史，总体感觉还是结缔组织
病，川崎病或者其他一些血管病变的结缔组织病。查查眼底血管看看有什么问题没。肾穿可能有帮助。但是
这是损伤性的检查，可能需要等到有必要再做。
最后提到的高血压，症状非常类似嗜铬细胞瘤，但嗜铬细胞瘤在儿童中不常见，最好能测一下尿VMA（香草基
扁桃酸）和metanephrine(变肾上腺素)。另外，neuroblastoma(神经母细胞瘤)有时也会一过性释放肾
上腺素引起高血压。当然高血压也可以是结缔组织病的小血管病变造成的。这个高血压的问题不像是中医可
以搞定的。
另外一些想法提出来集思广益吧：虽然没有证据，会不会有系统性的真菌感染？回盲区的肠壁增厚，会不会
有肠壁淋巴瘤(MALT lymphoma)？肠结核？中国的小孩都接种卡介苗，PPD阴性是不是说明有T细胞免疫缺
陷？患者有肾炎综合症的表现，但不知道有没有肾功能异常，水肿等。这么牛的广谱抗生素没有用，能不能
用实验性的抗结核治疗？实验性的抗真菌治疗？ |

发帖数: 3584 | 40 在网上查了一下，我的反应是第四种，但是每次照胸片儿都没有问题。卡健苗一定是打
过的，好象说我从小种这些东西的时候就反应超强。
(1)“+”：红晕及硬结，直径5～9mm。
(2)“++”：红晕及硬结，直径10～19mm。
(3)“+++”：红晕及硬结，直径大于20mm。
(4)“++++”：除红晕及硬肿外，还有疱疹或坏死或发热等全身症状。
现在的问题是医生要我吃九个月的药，说吃了以后再做皮试没有反应了，但这东西对肝
功能有很大影响，还要定期测血。再问别的医生，有的竟说卡健苗不安全且不可靠，皮
试的反应看我可能是带菌者，只不过没有发作。如果这样的话吃药又有个头用？但医生
的意思好象是吃九个月就没事儿了。
求高人指点。 |

问几个简单问题，不一定有帮助，但是可以考虑：
1. 有没有结核病史？多数是隐性感染都没有的。
2. PPD阳性还是阴性？阳性意义不大，但是阴性可以排除结核。
3. 痰培养如何？阴性意义不大，但是阳性可以提示结核。
4. 有没有PET-CT?如果阳性可以比较明确诊断肿瘤。
5. 肿瘤marker，包括PTH，CEA，CYFRA21-1等。阴性意义不大，但是阳性可以提示肿瘤。
仅仅靠一张CT片子有时候确实很难鉴别。 |

1. 没有结核病史，
2.ppd试验检查为阳性，但是你知道的，很多中国人都是阳性的。
3.没有做痰培养，正在考虑做。
4.没有做PET-CT，因为在2周之内，已经做了两次CT了，怕她吃不消。
5.查过一些血清因子，比如CD12为阴性。你所指的三种没有查。

发帖数: 150 | 46 我目前怀孕32周，三星期前开始发烧。1）一般是傍晚5,6点开始。38-38.5度。最高烧
到过39度。发烧时靠吃Tylenol降温。2）盗汗 3）发烧时喉咙痛，有时胸闷气短。
目前做了尿检，blood culture,结果都正常
respiratory virus panel：所有结果也都是阴性。
CBC： NEUT(中性粒细胞）偏高 7.7（1.6-7.2）
MONO（单核细胞）偏高 1.1（0-0.8）
IM GRAN（粒细胞）偏高 0.06（0-0.03）
CMP：有些值偏低，不过医生说在怀孕期间也算正常。
另外做了tb的皮试（PPD）结果阳性，做了胸部x光，无阴影。目前在等另一tb-test结
果。
目前我的infectious disease specialist也没有太好的解决办法，只是在等待。所以
我想请问贵版的朋友有没有人见过类似的病症，能够给我一个好的解决办法。 谢谢大
