it appears that you neglected to submit a copy of the actual x-ray report.
The civil surgeon wrote a summary statement regarding the x-ray report,
which is not the report itself. Please refer to Page 2, Item 4 stating a
copy of the x-ray report is to be submitted.
The medical examination report, Form I-693, submitted with your application
to adjust status, did not include a separate x-ray report. The form i-693
instructs the civil surgeon to attach a copy of the x-ray report when a
chest x-ray is required. Your original Form i-693 is hereby returned to you
in the enclosed sealed envelop. Take this letter and your enclosed form i-
693 to the civil surgeon who conducted your medical examination. ONLY THE
CIVIL SURGEON SHOULD OPEN THE SEALED ENVELOPE. The civil surgeon must attach
an x-ray report to the form i-693 and return both to you in an envelope
sealed by the civil surgeon.
X-ray report requirement: if you are required to receive a chest x-ray, the
civil surgeon must attach an x-ray report to the form i-693. USCIS will only
accept a full and formal chest x-ray report, whether a copy or an original,
signed by a physician and on official hospital or medical office letterhead
. USCIS cannot accept any preliminary or incomplete evaluation that does not
describe the full evaluation or findings. In most cases, the civil surgeon
will have to wait a day or two before this report has been sent by the
reviewing medical office and can therefore not sign off on the Form i-693
until this report has been received.

1。是不是说我要找给我做x-ray的医生要x-ray report，然后再去civil surgeon那里
重新做i-693表，让他把那份report和新签的693表seal后寄过去？
2。不知道再签一次i693要不要另外算钱啊。

告加进去再封个新信封就好了。不应该要加钱。 |
