各有利弊,现在普遍认为PA PRACTICE的范围更广,不象NP,如果是FNP只能是PRIMARY
CARE, ACUTE NP只能是ACUTE, PA就不存在这样的限制,而且PA可以不需要任何
CERTIFICATE就可以是手术第一助手,而NP需要额外CERTIFICATE 才行,当然PA没有处方
权,而NP有. |

医生SIGN一个AGREEMENT,意思说可以SUPERVISION下, PA 才有处方.如果你查一下
SALARY的话,PA平均值是比NP高的,正如前面说的,NP走的是NURSING MODEL, 属于
NURSING BOARD, PA 隶属于 MEDICAL BOARD.其实NP在医院的PRACTICE SCOPE 还是很受
限的,除非你既有FNPLICENSE, 又有ＡＣＵＴＥ ＮＰ不然LICENSE，不然你要么看不了
儿童,要么不能去ACUTE, 当然现在NP市场比较乱,不排除FNP在医院ACUTE 科室工作. |

的。掏心窝子的建议啊 ，去的学校的location一定要好，因为很多时候你的实习地方
就会考虑给你offer了。要是毕业以后再满世界的找工作特麻烦。2年10万，看你怎么想
了。acute NP 出来的年薪（在主要城市）怎么也有8-9万起，所以还是值得的。
sponsor的问题挺麻烦的，了解不多，听听其他人的建议。 |

发帖数: 218 | 6 我来说说吧。这边的透析护士分acute mobile dialysis and chronic dialysis.前者
要到不同的医院给病人做治疗，根据scheduler的安排，你一天可能要去2-3个医院。有
时候一个医院做一个病人，再去下一个医院做下一个病人。这个活得看公司怎么给护士
算钱。多数是paid by case，现在的行情大概是150-170刀做一个病人，少数公司按小
时算。如果按case算，透析护士必须要做的很快。我看到有些护士为了多干活，偷偷把
病人的治疗时间给掐短了，医生要求做3个小时的透析，护士会把时间掐到2个小时45分
钟。如果在同一家医院做3个case，可能只要12-13个小时就做完了。如果是公司按小时
给工资，这里面猫腻也很多，护士就可以适当磨羊工，反正我做3个case，可以拖到15
个小时。如果还要开车到不同的医院，开车的时间也是要付钱的。这样下来，3个病人
的时间可能要花到16-17个小时，如果12个小时以后是双倍工资，那这一天下来，工资
会很高。不少中国护士都做这个。比较适合单身，或者家里有另一半专职照顾小孩的护

NH 干了两三年.
interview 时那个 DON 说她们做 Blood transfusion, heparin lock, 还有呼吸机，
一个护士只照顾 7 个病人，她们 不做 rehab, rehab 都转到别的 rehab place 去做
。说得我很糊涂. 因为这个 hospital 就叫 rehab hospital 啊？ 我以前在的小的 NH
也有一个 subacute unit, 从来没有上面的那些，而且一个护士要照顾 20 多个病人。
这么说是不是这个 hospital 和别的 acute-care hospital 差不多了？我的目标还是
希望以后去一个 acute care 的 hospital, med-surg/ER/ICU 之类的unit, 希望这段
工作经历对以后有帮助。 |

CERTIFICATION 考试，现在版上读NP的越来越多，那么我就来抛砖引玉一下，谈谈NP
的认证考试。
大家都知道NP SCOPE，法律法规因为是州内来，所以NP 的执照也是州内颁发的，所
以NP 的考试并不是 BOARD OF NURSE 统一出题，而是交给了有认证资格的NON-
GOVERNMENT 来全权负责。一般来说大部分州都对 AANP 和 ANCC 承认，如果你是学FNP
，ANP （或 AGNP），PNP，那么这两个考试你可以任选其一就没问题，因为我学的是

HOSPITAL 也都不一样, 有的只收轻微病人, 有的还是收比较重的病人.ICU 肯定和普通
医院ICU 不太一样, 因为病人住的时间比较长, 但都有VENTILATION, 有时侯也会有药
的TITRATION, 毕竟它还是属于ACUTE CARE.我觉得那里ICU 更象IMC 这种, 对于新毕业
生还是值得去的 |

这两个是不同的training programs啊。
residency 侧重clinical practice. fellowship更侧重research. fellowship 一般都
是2年的。我对industry or academic 的fellowship 不大了解。医院的fellowship我
知道一些。我知道有的 fellowship position 如果没有招到人的话也有可能招 PGY2.
比如NICU, PK啥的，这样是一年的training, 不过实际上也是PGY2, research 成份相
对少一些。申请PGY2 position 需要先完成PGY1。
今年感觉申请 residency竞争很激烈。一般pharmD graduated from a good program
with high GPA (at least 3.5), and hospital working experience. 可以保证你几

retail的一个老pharmacist说现在买的病人很少了，就是因为前几年报道的多起acute
pancreatitis。他说这个药迟早会被拿下。
byetta应该降post prandial血糖很好，每天两次，饭前用。
但副作用一直争议很大，最显著的是nausea,争议的是acute pancreatitis and
thyroid cancer。
上课的时候老师说AP报道的30多例，其中28例是因为病人有其他的AP的risk factor
like high triglyceride, alcoholism...所以大家overestimate了药的副作用。
但retail的销量直接反映的临床的应用
不知道其他有经验的人怎么看byetta的使用呢？ |

闹了半天是个不看贴就摆架子，威胁人，骂人，删帖的版主。混mit这么多年，还真遇

in acute setting plus a normal MRCP. With somebody who has autoimmune
tendency, GI vasculitis may be considered in the differential. I agree with
A++, acute intermittent porphyria is possible and a urine porphyrins
including porphobilinogen can be easily done. I guess if all are negative,
she then might have somatization disorder. |

营养不佳的孩子（VitA 缺乏或者marginal VitA status) 易患麻疹或者其他呼吸道感
染疾病。我个人认为可能很多情况并不知道自己体内有没有充足的Vit， 所以我就提到
要保持VitA在体内的正常水平。健康人群如果饮食平衡，应该不会缺乏VitA. 我发出帖

5gigs available and puts data to the storage. The file table is stored in
the controler. So the controler puts data to the storage as long as he
thinks there is enough space. But in the storage the real data is just

My colleaugue asked me to discuss Zofran. Zofran (ondansetron) is a
frequently prescribed anti-nausea medication that works on the 5HT-2a
receptors in the area postrema of the medulla (the chemoreceptors that
trigger nausea) and is used quite frequently. Zofran PRN is on our standing
orders when admitting patients to the hospital. Although there are some
side effects, such as headache and sedation, the side effect profile is more
favorable than other anti-emetics.
