iron profile, ferritin level
我感觉不太像缺铁性贫血。因为
1）从报告来看，是小细胞贫血没错，但RDW正常，一般如果是缺铁性贫血，血细胞大大
小小，RDW会变大
2）血红蛋白13.2， 正常情况下红细胞是13.2x0.3=3.9,但你的是5.8，所以有点像地中
海贫血的样子， 如果你有以前的验血报告可以比较一下。地中海贫血应该从小到大都
是如此。
3）能引起小细胞贫血的，主要有4类，除了缺铁性贫血，地中海贫血，还可以是铅中毒
（小孩子多见)，或慢性疾病引起的
4）如果是女性，我就会怀疑是月经丢失引起的。但像你这样的成年男性，如果是缺铁
性贫血，除了摄入不足（一般正常饮食是不会缺乏的，除非你是vegan, 有十二指肠的
问题，那里是vit C帮助吸收铁的地方，或其他小肠的问题，慢性腹泻，比如celiac
sprue），那就是丢失过多，比如从消化道，或尿道（如结石）。 可以查个尿常规，看
看有没有血细胞，做个大便潜血试验。

drugs for maintain the function of his heart, including ACEi, B-blocker,
blood thinner & water pills. He is also taking Ferrous Sulfate, Folic Acid
and Multivitamin. So my question is what is the rationale that he has to
take all these multivitamins and minerals. I will highly appreciate that if
you can give me a more detail answer for each single supplement.

1.我们讲的是tips是一个bridge to liver transplant。做了这个以后就没有liver
funx。而这个病人做tips已经是3年前了。是不是可以用propanolol后考虑liver
transplant。
2.病人入院之前的folic acid和iron pill具体的indication是什么啊。这个case就是
要找出drug therapy problem。这是不是unnecessary medication呢·
3.因为pt已经dehydration，是不是要hold furosemide？那相应的HTX应该如何控制。
要不要增加lisinopril的dose。
4.我的理解病人之前take kcl是因为同时on furosemide。如果order lab之后，k
level在正常，是不是就可以也hold kcl呢
5.因为病人有pud 引起的bleeding，是不是也要加上在discharge的时候put pt on
life long ppi （high dose）？
