Second, a pt with asthma might be normal in PFT when he is not in attack,
although it will worsen gradually later without proper treatment.
Third, besides above data, some other findings might support the diagosis of
asthma, such as abnormal TLC, RV or FEF 25-75%.
Fourth, for the diagnosis of asthma in this pt, a metacholine challenge test
might be very helpful.

question list, so PN is more important for data gathering score. When I took
exam in philadelphia, patients tended to volunteer information.

心率飙到了190。一打开查询成绩的界面，映入眼帘的是一行红字，像有当头一棒，第
一反映是：CS没过？还好定睛一看，You have met all of the requirements for
ECFMG certification. 心头的大石放下了。
回顾那疯狂的半年step1+step2 CK CS+生娃，真是感慨万千，特发文纪念。按流水帐纪
录吧。
3月份-4月份：step1
怀孕中期，正在慢悠悠地看着usmle的书，等待8月份宝宝的诞生。之前的2年，已经过
了一遍书，慢悠悠地过了一遍kaplan，正确率是70%左右。不料，match结果出来后，傻
眼了——LG match去了外地。几番考虑后，决定速战速决，尽快考完。NBME模考220。
于是报名了4月18号的考试。在接下去的一个月，开了UW题库，每天2个，晚饭后开始
review题目。刚开始的时候，一个block大概要3-4小时，所以，往往学习到凌晨2点。

一下大部分文字是考前两月写下的自己的感悟，由于考前高压，可能会条理不清，词不
达意，见谅。
不变应万变
对于难题要做到镇静，想想我不会的题目，多数人也未必会，镇静，镇静！镇静下来之
后才能慢慢寻找解题思路，难题的难点在于，题目信息不熟悉，备选答案内容不熟悉，
不要慌，想一想，出题者的思路在哪里，用自己熟悉的知识点去解决不熟悉的信息，这
就是apply的过程，比如genetic的题目。以不变应万变。
对于不会的题目，不要粘题，没有思路，跳过去，回头再想。有时是瞬时block out一
叶障目，后头再看的时候会发现豁然开朗。不值得浪费太多时间在一道题目上面。
关于排除法的应用，首先排除自己熟悉的选项，再排除自己不熟悉的选项，如果自己熟
悉的选项不能排除，怎么能排除自己根本看不懂的选项。解决方案：如果最后自己熟悉
的选项不能排除，还是选择自己熟悉的选项。因为太超范围，太偏太远的东西考的可能
性毕竟比较小，较大点的可能是可能是自己熟悉的，但是自己没有做到很好的apply或

发帖数: 68 | 16 4月下旬亚特兰大考试，本周三出成绩，PASS（CIS星星，SEP比较好但没星星，ICE
borderline上一点点）
很怕CS不通过，所以花了好些钱。参加了Kaplan的Newark 5天班，考前模考班。参加了
Dr. Puma的21天班和考前模考。
考完第一步后很快和考友将FA的大case过了一遍，没有练习PN和PE，就去参加5天班了
，我不想浪费时间自己摸索，花钱买个捷径。上课收获很大，系统地全面基础地了解了
CS这个考试。个人觉得kaplan的CIS足够好，PE教得也够用够好。SEP模拟成绩可以反应
自己的口语如何，但是ICE绝对是要再自己回去花功夫强练的。
参加Kaplan班时住的是Best Western Plus Robert Treat Hotel（50 Park Pl，Newark
，07102 NJ）。住的条件一般，但还比较方便也便宜，有专门的shuttle接送。
5天班最后一天的模考成绩很差。PE和PN完全没练过， ICE离Kaplan及格线有一定距离
。SEP和CIS还好，基本过线。我没有实际临床经验，也没有考CK，通过这个模考也确定

我绝对是毕业很多年的啦（我比Dr. Coffee早几年医学院毕业，做了很多其他的事情，

在去年和今年考的(1st attempt)。我同时申请两个专业（外加prelim),现在有十几个
IV，我去了三个IV。我的问题是我申请的专业有一些是advanced program（尤其是我很
想去的），可我的prelim IV只有三个。
我个人认为我的交流能力还是可以的，因为我的日常科研工作需要大量的交流（而且
我也工作很多年了）。我认为我IV最大的问题是进一步把我的以前的工作经验和临床更
具体的联系起来，特别是证明我的以前的经验会让我胜任住院医（尤其是PGY1 intern

不需要完美的英语，12个病人，10-12分钟交流，内容很熟悉，目的：得到你想要的信
息，诊断，病人想说的信息问出来；需要病人听得懂（简单的语言，语速不要太快，模
板，避免she/he, hands, tense这种错误，不用复杂的单词，地道，语调，语气：同情
等，不要像背书，脱口而出，跟情景语气等符合），病人的话听得懂（100%注意力，同
时要做笔记），避免彼此的重复，注意病人是否听着费力，不要经常用一种开头或
response，但是仍然要简单
CIS: 听懂病人情绪变化，病人暗示了，一定要接上给response, non-verbal, 打招呼
，眼神，坐得比病人低（病人心理出于劣势，仰视病人，不要站着- 感觉rush，没有足
够的关注），关心，身体前倾。情绪变化很subtle，要察觉到，安抚等。

在医院看病人，然后再回家看书是一件有意思的事情。白天收了个肾盂肾炎的病人，晚
上回家把尿路感染的章节看一遍，会有“原来是这样啊”的顿悟。尤其是病史的部分，
书的描述经常和病人的话惊人得一致，这种感觉很奇妙。第二天再看病人，会自然地注
意到书里的细微的症状和体征，就会有水平在短时间内突飞猛进的错觉，自我感觉会很
好。
病房的病人多是重复的，同样的病人也大多雷同，但是每次读同样的内容都会有新的体
会。这个估计就是“温故而知新”。印象深刻的是一个主动脉瓣狭窄的病人。晚上监护
室大妈呼吸困难，血压很低，人说话说着就不行了，利尿也没用，去甲肾上腺素也不管
用。回家一翻书，原来就是主动脉狭窄病危时候的死亡漩涡。大妈危及的情况被书上平
淡的几行字描绘得相当逼真。之前也看过好几遍，但是从来没有这种感觉。忘记是哪个
主任说过，书上每个字出现都有原因，背后都是病人生死的故事。这个是真的。
然后再说看什么。我是看不了UpToDate的。里面内容太枯燥，太多数字和文献。我感觉

在我申请residency之前，实际我想做pediatric的，后来Match到内科。工作中，我觉
得我很喜欢老年科。
1） 其实和小孩子一样的，有的老人无法告知他的感受。
2） 老年病人需求多的交流，我们的病人更appreciate 你和他talk。
3） 然而，更多的是和病人家属交流。跟家属交流的艺术，在IM training的时候很
少，在老年科的时候就多了。
我现在做fellowship的时候，有很多training的机会。比如前段时间我们做了
Simulation training，给了个病人，肝硬化晚期，已经不能够liver transplantation

患者44岁，男，6年前因为肝癌做了肝移植 （符合米兰标准），之后无灾无病好几年。
去年2016年初发现轻微贫血，血色素12，因为当时在服小剂量阿司匹林，于是停服，看
看有没好转。托托拉拉到了年底，还是贫血，血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤，内镜医生直觉是GIST。随后CT，但CT却没有异常。4月初第二次胃镜
，GIST依然还在，取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

了,肯定比你那个FATIGUE是PRIORITY,我感觉你对NURSING DIAGNOSIS的POINT还是没抓
住,不是病人现在稳定,她的PRIORITY就放在那些她现在现有的问题上,Preeclampsia还
是很凶险的,头十分钟稳定,后十分钟可能胎儿就宫内窘迫了,胎儿的缺氧也不是单纯
late deceleration能看到的,PREECLAMPSIA对胎盘造成的影响是逐步的,不断的.你休息
一下,再好好想想是不是,GOOD LUCK. |

的书哇？那个书上一般写的挺详细的，你就找一个diagnosis，然后往下写就好了，有
模板啥的我记得，要是是真正的病人，参考病人档案里其他护士写的。还有问问老师究

是两种diagnosis的 sensitivity吗。test pb=pc 不是正可以回答两个diagnosis方法
的sensitivity相同不相同的问题吗。如果不相同 那末就看看那个高 那个就好。或者
用one sided test。
