其实很多读NURSING的都是有科研背景的，远的不说，THR就是以前读PHD的。以前还有
cell发了第一作者的转了NURSING，还有国内TOP2，美国芝加哥大学的博士读了NURSING
的。
读NURSING的女性居多，很多就是跟着老公出国，老公往往都是博士，博士后。所以不
要拿科研说事，那个东西没有啥神秘的，其实跑PCR就和RN打静脉一样，都是熟练工种
，没有什么高大上的。
而且科研这个东西，也要做对方向，科研就是挖矿，如果方向对了，就是事半功倍，不
然就是千老一辈子。
认识一个台湾人RN，自己有个哥哥就是数学系教授，还被中东的dubai的大学
请去讲课，带博士生什么的。 她就是一个RN，天天就在医院做一点paperwork，工
作轻松愉快。两个儿子，都是她帮忙在本地富人区给了首付，一个儿子是律师，也
娶了律师，买的三百万美金的大房子，然后两个孙子，儿媳妇买投资房都是那种一个月
物业费九千美金的。
还有一个儿子也是她给的首付在本地富人区买了房子，现在自己也是三套房子。
这个台湾RN从来也不加班，老公也只是一个牧师，教会的牧师没有钱的，一年最多四万

产生肺水肿的重要原因及其后果：
↓Cardiac output-->↑end-diastolic volume & pressure，↓urinary output -->
pulmonary edema -->↓ ventilation & gas exchange --> hypoxia
针对这个病理，临床上的药物治疗目的主要是促进ventilation & gas exchange，增加
cardiac output。而要达到治疗的目的，除了输氧外，一方面是增加排尿量以减少不正
常地潴留于肺部的液体 — 通过利尿剂 &/or 增加cardiac output；另一方面是增加
cardiac output — 通过降低血压、减少心脏的workload及增加心脏的收缩力 — 如果
这个目的达到了，问题的根源就得到了解决。
从题目来看，在排尿量达到预期值、及pulse & RR偏离不大的情况下，降低血压就大概
成为了选择的关键，而且，C中的血压值也在正常范围内、未达到低血压的程度。另外
，我也在想，如果排尿量过大，也可能会导致hypovolemia --> hyp |

number of ways to examine cardiac perfusion, and all of them have advantages
and disadvantages.

发帖数: 389 | 39 比赛从Folsom湖边的Beals Point开始。出发前在队伍中看到我们南湾的高手Toshi。我问他今天的目标是啥？他很客气地说争取跑进20小时。我当时觉得他太保守，要知道他可以3个半小时跑完50公里的比赛。他的陪跑整容也无比强大。除了父母妻子外，竟然还有去年这个比赛的第二名Sean来做pacer。最后Toshi得了第三名，成绩是20小时53分21秒。我后来看了他今年的比赛报告，感慨高手也一样可以跑得很艰苦。
比赛5点2分开始。外面依然一片漆黑，还需要使用头灯。先是一个2.26迈、沿bike
path的out and back 。我尽量将自己的pace控制在12分不到一点。在1.19迈处掉头时
，我发现后面没剩几个人。由于pace还是有将近12。我深呼吸了一下，默念了跑版上学
到的教诲数遍：他强由他强，清风拂山岗。比赛还很漫长，急不得。
几乎每个ultra的老兵都会和你说，100英里的比赛一定要慢跑。但是能做到的真不多。
因为我们这次比赛，大部分都是久经沙场的老将，开始照样个个跑快。可能大家刚
taper完，浑身是劲，12分的pace简直是慢得无法忍受，或者说没办法跑这么慢。但是

发帖数: 389 | 40 离开Auburn 供给站，先是沿着公路跑一段，然后回到了那条小渠。大热天谁不喜欢潺
潺的流水？用清凉的河水打湿身体，我忘记了烈日下的炎热，也想起了小时候偷学游泳
的顽皮。回去是顺流，缓缓的下坡跑起来很爽。中间有一段坑坑洼洼、朝山下倾斜得很
厉害。斜坡上有一条凳子，上面画了一张很大的笑脸。有点想我家小丫头的画，好可爱
的。
沿小渠跑了不到半个小时，接下来是陡峭的下山。下山如果很陡，行进速度比上山还慢
。我小心翼翼地迈着每一步，生怕一个不小心摔倒。下午5点32分抵达位于50.19迈的
Cardiac Hill 。
经过12个半小时的奋斗，革命任务终于完成了一半。我换了鞋袜，重新穿上那件搞笑的
“Where is the Finish Line”的T-shirt 。我喜欢那种到处乱找不知道终点线在哪里
的傻冒形象。将USB充电器插在手持水瓶的口袋里，重新开始给Garmin 305充电。
自我评估了一下。身体各方面都感觉良好，除了有些累，没有发现问题和隐患，不过也
不大可能比前半程快。如果想25小时完成，需要后50迈跑出和前50迈一样的速度，太难

发帖数: 389 | 41 晚上10点过1分离开Twin Rocks（65.16M），再次前往Horseshoe Bar（89.10M)。临出
发时，有人朝我喊：“Only 35 miles left!”。我苦笑了一下，是啊，“只”剩下一
个50公里的超马，还外加一个5K。跑100迈的比赛真的不能算剩余距离，会算得人好沮
丧。在那个没有标记的路口，我记了一下GPS的数字。接下来我每遇到人，就告诉他/她
：最后0.6迈几乎没有标记，到前面多远多远的路口，右转后左边第二颗树有丝带，等
等。我先后至少和7拨人说过。天亮后路上有人和我打招呼后谢我，说昨晚要不是我，
她很可能会迷路。我听了真的好有成就感。
这回程的6.07迈花了我2小时8分，比来的时候还多32分钟。基本上是走得多跑得少。记
得王欢喜说，100迈的比赛从70迈开始。我这还没到70迈就已经慢得象砣shi。也不是人
有多累，腿也还有力。聊天固然花了些时间，更多的还是糟糕的路况，实在太让人缺乏
信心了。
一路上遇到对面来的好多熟人。咱们的老中哥们看起来很不错，一定可以完成他的第一

我时不时来生物版看看，因为我来北美也做了10年的科研，我自己觉得我有兴趣但
没有大的出路，也不是我和我的家庭的晚年的理想归途。于是，我40岁开始做跳槽
努力，虽然勉强过了USMLE，混进了住院，但我尽了最大努力做好，近50岁时从新开
始当医生。至少，我和我家人不再为养老发愁了，这里就不说那些悬壶济世治病救
人的高尚话语了。
我就不明白，要是真对科学感兴趣，愿意献身，老哭诉什么？如果不甘心，不请愿，
那这个自由世界有得是机会，你自己去努力总有可能改变自己不满意的现状的。即
使最差，你总是去努力过了。
人家老墨干餐馆、加油站，清洁工，也嗨皮得很，更有俺翻译的这个故事的精英。
而在这个各个牛B哄哄的大陆精英版里，看到的就是鬼哭狼嚎抱怨不休。俺来几次贴
点鼓励和建议的帖，总有那几个牛B到连俺的帖子的目的都看不懂，愣要说俺来显摆
暴发户的。
靠，就尔 等这水平，估计也就配在LAB里哭鼻子牢骚养耗子跑PCR了。
俺从来都是真人上网，当个普普通通的病理医生很满足，但我深知作为出来的学医
