但是都是需要找guideline，按照guideline的要求，
1，相机跟人的距离。
2，出来的照片按照guideline要求，取头像，照片的大小，人头像的大小，眼睛到相片
底的高度等等。
然后到costco洗。
从来就没有被据过。
（当然，当初浪费了很多照片）
照片拿回来了，按照guideline自己用尺子量一下。
熟练了以后就不需要了呵呵。 |

Acinetobactor species是常引起late onset HAP的细菌。
问问医院有没有查pseudomonas和MRSA，这也是常引起HAP的病菌，在治疗方案中也常常
对用上抗pseudomonas的抗生素。pseudomonas或者MRSA感染后果可能会比较严重，而且
只有某些抗生素才能抑制这些细菌。一定要问医院有没有查这两个细菌感染的可能性。
非常震撼！医院居然把病人直接送回家让自己康复？ HAP的死亡率是20-50%，如果
empiric antibiotics 没选对，死亡率对增加2-3倍！只听你说病史，还有细菌的确认
结果，多半就是HAP。病人有其他的症状吗？发热？白细胞数量变化？氧饱和率？是确
诊了还是怀疑HAP感染？
第二，用药问题。你问这个药不是推荐治疗HAP的用药。美国传染病学最权威的
guideline在
IDSA上。

留学生是415一个学分，绿卡减半，下学期好像又要涨一点，
suny是州立系统，cuny是市立系统，
我一个同学BMCC毕业的在我们program，
他BMCC用的paper都可以拿过来接着交，
每次只要挖挖以前的paper拿来交就行了，
说明ASN和BSN很多课其实都差不多的，
就是听说suny downstate学校那边很多黑人，
你实地跑一次去看看，但是他们有自己的medical center，
我选hunter因为地铁方便，在曼哈顿，大部分都是白人和亚裔，部分黑人，
SUNY他们那个program prefer有绿卡的学生，要申请要尽量早点。
RN－BSN Nursing的课都是每学期几个paper APA format，
老师都会提前告诉你要写什么，
最多就是有个期中考，有些没有期中考，
期末一般没有考试，平时就是听老师讲课，
看看老师给的文章，做几个online的certificate考试，
最多一个paper，然后期末就是group presentation。

读完“让哥来说说国内国外的医疗对比”有一个感觉，就是笔者没有在国内医院工作
过，也感到很多对中美医疗的评价有些问题， 我不管对美国的评价如何， 但特别
关注对中国的评价， 因为我一直认为这种评价， 特别是来自这些高学历有着中美背景
的评价特别对国内医疗的改变有益。（我出来已有好几年，国内医疗最新改变
不清， 很希望比我出来时该别很多，不仅仅是仪器改变）
看到文章里谈到好医院的名医， 我感到有些好笑， 不是笑作者， 而是笑自己，
自己很幸运，出来前就晋升副高几年 如我不到这儿来， 现在肯定被认为是“名医”
了，还是省会数一数二医院里，数一数二的科室里的“名医”， 我自己也肯定

事情发生的太突然了。四天前我和先生去看望他们还说要这周末一起庆祝我成功match
，没想到这一见竟成永别。。。也打乱了很多计划。
请问这里，尤其是内科的前辈因为腹痛而收入院的常规guideline 在哪里找？我找遍了
ACCF/AHA guideline，里面只有确定病因以后的guideline and protocol，是我找的不
对？除了手里有本18版Harrison's Internal medicine, 还到底应该去哪里找这方面的

战时新闻报道的客观性一直是美国的新闻传播研究中的一个热门话题。研究最为集中的应
该是在新闻自由和政府干涉的关系上，这个从越战之后变化非常之大。
研究领域普遍认为越战的报道是fair的，一个重要原因是当时的政府干涉并不严重。政府
干预一般有三种形式，rules/guidelines for media coverage，审查制度(censorship)
以及军方对媒体行动范围的限制。其实越战报道也分两个时期，在1968年的Tet
offensive以前，记者的行动基本是依据军方制定的agenda，据当时的新闻工作人员说，
也有一些guideline，而且记者与军方有一定的默契，有些关系到国家安全的不能写。这
时候的报道大多数集中在美军如何progress上面。转折是1968年的Tet （越南新年），越

第一句的意思是这个Configuration有向Guideline方向努力的意向，但是失败了，没达
到。因为有Fail,那么就有Success，隐示达到guideline就是成功，反之是失败。
第二句就是阐述了这个Configuration按照这个Guideline的标准来看没达到。只是呈述
事实。
举个例子，一个中国人英文不好，只是一个事实，像句2一样，中国人中文好就行了，
英文好不好无所谓；一个中国人，想到美国读书，但是TOEFL考砸了，所以英文不好没
能被录取，这说明到美国读书需要英文达到一定标准，想达到这个目的的人如果没达到

你如果去看自己的PCP，如果满足guideline的话，可以测
我自己的PCP guideline是排除flu和strep throat后，101.5度，cough和chest pain/
shortness of breath，没有travel和contact的限制。

这些guideline只本地的health commissioner定的，CDC的建议只是建议。纽约这种
epicenter的情况不能拿来说是全国的情况。很多地方测试很容易，我知道有大农村
的drive through只要发烧就能测。

我必须承认我的博士和胆固醇没有太大关系，所以实在不是这方面专家。但是好在你说
的这些文章我能看懂（前世修得好，感谢佛祖）。那么我甚至还能看懂其它相关的资料
（前前世也修得好，感谢观音），就说几句评论：
（１）Statin，俗称降胆固醇药，不是一种药，而是一类药。有几个已经被勒令撤出市
场，因为副作用很大。
（２）Statin 对于所谓坏胆固醇的降低，是治标不治本的，虽然通过降低坏胆固醇的

GUIDELINES but also stipulate their own OVERLAYS (requirements).
So, most lenders will not use others' appraisal report. This does not
violate any GUIDELINES ( which say reports can be transferred but not must
be transferred) so they can still sell loan later to secondary market. But
